15 - Laparoscopic Considerations in Pediatric & Adolescent Gynecology

WEBVTT
00:00:00.239 --> 00:00:05.280
A 13-year-old who presents with an adenexal mask requiring surgical management.
00:00:05.280 --> 00:00:10.560
What should we think about and what do we need to know before heading to the OR?
00:00:10.560 --> 00:00:21.039
Welcome to Pag Over Pastry, which is a 20 to 30 minute podcast reviewing very important topics in pediatric and adolescent gynecology.
00:00:21.039 --> 00:00:37.359
Today we'll be reviewing laparoscopic considerations for the pediatric and adolescent population based on the Essentials of Pediatric and Adolescent Gynecology published by NASPAG and authored by Alexandra Adler and Patricia Hugelett.
00:00:37.359 --> 00:00:43.679
I'm Susan Kaufman and I'm a Pediatric and Adolescent Gynecology at Virtua Health in South Jersey.
00:00:44.000 --> 00:00:44.399
I'm Dr.
00:00:44.399 --> 00:00:45.439
Olga Kciuk.
00:00:45.439 --> 00:00:49.119
I'm a pediatric and adolescent gynecologist at Stanford University.
00:00:49.119 --> 00:00:50.479
Thanks so much for having me.
00:00:50.799 --> 00:00:54.560
Well, thank you for joining me on this very important topic.
00:00:54.560 --> 00:00:56.799
So, what's your favorite pastry?
00:00:56.960 --> 00:00:58.560
Ooh, I'm gonna go a little niche.
00:00:58.560 --> 00:01:02.399
I'm gonna say a cunha man, which is a French pastry.
00:01:02.399 --> 00:01:05.840
It's buttery, it's caramelized, it's fantastic.
00:01:06.159 --> 00:01:07.599
Let's get started.
00:01:07.599 --> 00:01:15.439
So let's start with some pre-op considerations.
00:01:15.760 --> 00:01:16.319
Definitely.
00:01:16.319 --> 00:01:26.480
So when we are working with our pediatric and adolescent patients, thinking about informed consent needs to come hand in hand with thinking about assent as well.
00:01:26.480 --> 00:01:32.719
So we obtain informed consent from the legal guardian, but we want to involve the teen.
00:01:32.719 --> 00:01:34.319
It's empowering for them.
00:01:34.319 --> 00:01:39.920
It helps to ensure that they understand the procedure, that they feel included.
00:01:39.920 --> 00:01:48.000
And it's important for us as providers to understand the laws around emergencies, around emancipated minors.
00:01:48.000 --> 00:01:50.079
These can vary by jurisdiction.
00:01:50.079 --> 00:01:53.120
So it's really important to know your own local rules.
00:01:53.439 --> 00:02:04.640
One of the things I do is I actually have the teens, even as young as 11 and 12, sign the permit, and then the parent signs next to that.
00:02:04.640 --> 00:02:14.879
So I really feel like I'm involving my patient in the consent to avoid the idea that we're doing something to them versus taking care with them.
00:02:15.280 --> 00:02:16.159
Definitely.
00:02:16.479 --> 00:02:28.479
And because our patients tend to be smaller, what are some anatomic considerations we have to keep in mind when we're going to perform laparoscopy in a pediatric or teen patient?
00:02:28.800 --> 00:02:31.840
Yeah, we know that kids and teens are not just small adults.
00:02:31.840 --> 00:02:37.840
So pediatric patients have some differences in anatomy that we need to keep in mind and be aware of.
00:02:37.840 --> 00:02:47.599
So longer fallopian tubes, higher ovaries out of the pelvis sometimes, smaller uterus, and the bladder also is higher in the abdomen.
00:02:47.599 --> 00:02:49.360
It extends more cranially.
00:02:49.360 --> 00:02:55.599
So thinking about laparoscopic entry, too, we know that the integrity of the abdominal wall changes with age.
00:02:55.599 --> 00:03:01.919
So fascial wall tension and strength increasingly progress through childhood.
00:03:01.919 --> 00:03:09.039
So the entry forces that you need to enter in an adolescent are greater than for a young child.
00:03:09.039 --> 00:03:15.680
And that abdominal wall strength is greater in an adolescent often than even in adults.
00:03:15.680 --> 00:03:26.080
We need to be really careful with entry because the other consideration is that the distance from the abdominal wall to the major vessels is shorter than in adults.
00:03:26.080 --> 00:03:33.759
And so the risk of injury can be increased if port placement and abdominal entry aren't adjusted carefully.
00:03:34.080 --> 00:03:39.280
Now, before we even get to the OR, we generally have some pre-op imaging.
00:03:39.280 --> 00:03:44.000
We generally start with a pelvic ultrasound that's done abdominally.
00:03:44.000 --> 00:03:59.120
We don't commonly do transvaginal ultrasounds, definitely not on children and generally not on younger teens, maybe some of our older teens who might already be sexually active, but only if it's absolutely necessary.
00:03:59.120 --> 00:04:17.759
And then depending on what we find and depending on why we're doing the surgery, we may go on to obtain an MRI to help with planning the surgery, especially for anomalies, including uterine anomalies, vaginal septums, vaginal agenesis, and so forth.
00:04:17.759 --> 00:04:37.519
And a trick that I just learned by reading this chapter is that you can place a vitamin E capsule at the introitus, which I guess will show up on MRI, and it helps the radiologist to measure the distance between the introitus and the obstruction.
00:04:37.519 --> 00:04:39.920
I used to use vulvar magnets.
00:04:39.920 --> 00:04:46.000
My radiologist would put a magnet on the vulva, but this is much better to put a little vitamin E capsule.
00:04:46.639 --> 00:04:58.240
Yeah, it shows up pretty clearly on the MRI, and it is great as a marker to help exactly delineate that distance between the introitus and the obstruction itself, and that can help a lot with surgical planning.
00:04:58.560 --> 00:05:02.639
Now, do we need antibiotic prophylaxis for kids and teens?
00:05:02.959 --> 00:05:05.279
It depends, the same way it depends for adults.
00:05:05.279 --> 00:05:16.800
So antibiotic prophylaxis, we know that it reduces the risk of surgical site infections, and it's recommended for clean contaminated cases that involve entry into the genito-urinary tract.
00:05:16.800 --> 00:05:26.879
Surgeries that only involve the adnexa, though, like an ovarian cysteomy, like we're talking about for our patient in this case, would not require antibiotic prophylaxis.
00:05:26.879 --> 00:05:37.680
When we're thinking about antibiotic prophylaxis, a few things to think about for children and teens is that we use weight-based dosing in pediatrics.
00:05:37.680 --> 00:05:47.439
Generally, if the patient weighs more than about 40 kilograms, then standard adult dosing can be used because that's the point at which they tend to kind of even out.
00:05:47.439 --> 00:05:54.240
One thing to know is we generally avoid fluoroquinolones in pediatrics, and that's because of concerns about arthropathy.
00:05:54.480 --> 00:05:54.800
Right.
00:05:54.800 --> 00:06:01.600
Although the data on that is sparse, and we could certainly use more data because it's a great antibiotic.
00:06:01.600 --> 00:06:09.600
And one of the other things we have to think about now is VTE prophylaxis, and that's become a big topic.
00:06:09.600 --> 00:06:14.879
So we can lower the complication rate of venous thrombosis in our operative patients.
00:06:14.879 --> 00:06:28.720
I know my institution has a form that we have to fill out, whether it's for outpatient or inpatient surgery, to either support not giving any sort of prophylaxis or what prophylaxis we're going to give.
00:06:28.720 --> 00:06:39.920
So routine chemoprophylaxis is usually not needed in adolescents under the age of 13, because there's a much lower risk of thrombosis in that population.
00:06:39.920 --> 00:06:48.000
I tend to think that that applies to older teens as well, unless they have risk factors for a thrombosis.
00:06:48.000 --> 00:07:02.079
And those risk factors may revolve around obesity if they're currently on an oral contraceptive pill, if they have mobility issues, if they have a known thrombophilia, a malignancy, or congenital heart disease.
00:07:02.079 --> 00:07:11.120
The reason that children have a lower risk of thrombosis is that there are physiologic differences in their hemostatic system.
00:07:11.120 --> 00:07:23.279
So they may have lower circulating vitamin K-dependent clotting factors, higher levels of specific thrombin inhibitors, and an overall lower ability to produce thrombin.
00:07:23.279 --> 00:07:27.920
So all these things help to protect them from developing a blood clot.
00:07:28.160 --> 00:07:35.199
Yeah, and if we ever are using chemoprophylaxis, then we would use like a low molecular weight heparin.
00:07:35.199 --> 00:07:37.199
That would be preferred over heparin.
00:07:37.199 --> 00:07:40.240
It has a little bit more predictable pharmacokinetics.
00:07:40.240 --> 00:07:44.079
There's no need for serum monitoring or minimal need for that.
00:07:44.079 --> 00:07:47.680
And it doesn't tend to have a lot of interactions with other medications.
00:07:47.680 --> 00:07:51.199
It's also very easy to administer subcutaneously.
00:07:51.519 --> 00:07:54.720
Now, what about anesthesia considerations for our patients?
00:07:55.120 --> 00:07:58.480
Broadly speaking, it is about collaboration here.
00:07:58.480 --> 00:08:07.600
So it's really important, as in a lot of aspects of pediatric and adolescent gynecology, to work with our multidisciplinary providers.
00:08:07.600 --> 00:08:15.839
And in laparoscopy, it's important to work with an anesthesia provider who's experienced in pediatric and adolescent physiology.
00:08:15.839 --> 00:08:21.040
The CO2 insufflation can impact physiology differently than it does for adults.
00:08:21.040 --> 00:08:28.399
But generally speaking, patients with cardiac and pulmonary disease histories that we need to be especially careful about.
00:08:28.399 --> 00:08:37.519
And so that would be a potential contraindication to laparoscopic surgery, as the pneumoparitoneum would increase pressure on the diaphragm.
00:08:37.519 --> 00:08:40.639
It can cause cephalic displacement of the diaphragm.
00:08:40.639 --> 00:08:43.600
And that can all compromise venous return as well.
00:08:43.600 --> 00:08:51.919
So patients with underlying cardiac or pulmonary disease should definitely be medically optimized before any planned procedure.
00:08:52.320 --> 00:08:52.559
Yeah.
00:08:52.559 --> 00:09:07.039
And that's so important since fortunately, so many young people who are born with complicated cardiac anomalies are thriving and living on to their teenage and adult years.
00:09:07.039 --> 00:09:12.879
So we always have to keep that in mind that these people need additional considerations.
00:09:12.879 --> 00:09:16.720
So now we've moved our patient into the operating room.
00:09:16.720 --> 00:09:32.480
We've taken care of their antibiotic prophylaxis if they needed it, BTE prophylaxis if they needed it, collaborated with our anesthesiologist, and let's talk about positioning on the table and examination when they're asleep.
00:09:32.799 --> 00:09:38.799
A thorough exam under anesthesia of at least the external genitalia is always recommended.
00:09:38.799 --> 00:09:50.720
And this can offer the opportunity for identification of variations of normal anatomy, so hymenal variants, for example, that might not otherwise have been picked up.
00:09:50.720 --> 00:09:55.840
We don't tend to use a speculum, and certainly it's not recommended for pediatric patients.
00:09:55.840 --> 00:10:02.399
If there is any indication to visualize the vagina and the cervix themselves, then we would use vaginoscopy instead.
00:10:02.399 --> 00:10:08.639
We would use fluid to distend the vagina, just like in hysteroscopy or cystoscopy to visualize those internal structures.
00:10:08.639 --> 00:10:15.039
Otherwise, when we think about uterine manipulators, those are rarely needed in pediatrics when necessary.
00:10:15.039 --> 00:10:20.879
We might use like a sponge stick placed in the vaginal canal, maybe a small Hulka mobilizer.
00:10:20.879 --> 00:10:32.320
Those can be substituted to allow for just a smaller manipulator if any of that uterine manipulation is necessary, which sometimes for some ednexosystectomies or that kind of procedure can be helpful.
00:10:32.639 --> 00:10:38.000
Why do we try to avoid using uterine manipulators on children and younger teens?
00:10:38.320 --> 00:10:41.679
We're just trying to minimize the risk of trauma to the tissue.
00:10:41.679 --> 00:10:47.200
For kids who are prepubertal, we have to keep in mind the hypoestrogenic state too.
00:10:47.200 --> 00:10:51.840
And that small uterus, there's that risk of uterine perforation.
00:10:51.840 --> 00:10:56.399
And we have to keep that in mind when we're determining the need for a manipulator.
00:10:56.399 --> 00:11:05.840
As we get to older teens, though, we have the options of using typical uterine manipulators, similarly to as in adults.
00:11:06.159 --> 00:11:08.480
Then the next step is surgical prep.
00:11:08.480 --> 00:11:13.519
We will generally prep the external genitalia and the abdomen.
00:11:13.519 --> 00:11:23.600
If we do not need to enter the vagina for any reason or operate on anything on the cervix or the vagina, we don't need to prep that area.
00:11:23.600 --> 00:11:32.320
If there is a need for a vaginal prep in a pediatric patient, we can actually use a syringe to instill the prep solution.
00:11:32.320 --> 00:11:34.720
So it's like irrigating the vagina.
00:11:34.720 --> 00:11:46.960
I have used Q-tips, which I can insert without hymenal trauma and prep the vagina that way, particularly because in pediatric patients, the vagina may only be four or five centimeters long.
00:11:47.200 --> 00:11:49.840
A bulb syringe is my favorite trick for that.
00:11:50.159 --> 00:11:50.480
Yeah.
00:11:50.480 --> 00:11:54.720
And we can use betadine if there's no allergy to betadine.
00:11:54.720 --> 00:11:57.600
And we can also use chlorhexidine.
00:11:57.600 --> 00:12:07.759
We can use chlorhexidine on the vulva and on the abdomen, and we can use chlorhexidine with lower concentrations of alcohol in the vagina.
00:12:07.759 --> 00:12:17.919
But I want to just mention that that is an off-label use for chlorhexidine, but it is safe to use in the vagina with lower concentrations of alcohol.
00:12:18.240 --> 00:12:18.639
That's right.
00:12:18.639 --> 00:12:22.799
While we're down there prepping, we can think about our foley catheter as well.
00:12:22.799 --> 00:12:26.720
So what are some considerations for foley catheter sizing?
00:12:27.039 --> 00:12:35.519
Well, we have to keep in mind that these are smaller urethras, so we are absolutely not using an adult-sized catheter.
00:12:35.519 --> 00:12:44.000
For children ages 5 to 12, we will tend to use a 10 French catheter or maybe even a smaller one.
00:12:44.000 --> 00:12:50.080
Some people go by the age of the child and then equate that to the size of the catheter.
00:12:50.080 --> 00:13:02.639
We sometimes need to fill the bladder so that we know exactly where we are anatomically, or use that marker for whether we're doing vaginal surgery or intraabdominal surgery.
00:13:02.639 --> 00:13:09.759
And I know for my pediatric and adolescent patients, I rarely, if ever, use a catheter.
00:13:09.759 --> 00:13:12.799
I have them void before we bring them back to the OR.
00:13:13.120 --> 00:13:15.120
Let's talk about patient positioning.
00:13:15.120 --> 00:13:20.240
As long as you don't need vaginal access, which as we talked about, you most often don't.
00:13:20.240 --> 00:13:23.600
Younger pediatric patients can be placed in supine position.
00:13:23.600 --> 00:13:28.639
If you do need access to the vagina, you could consider frog-like position for younger patients.
00:13:28.639 --> 00:13:35.039
And then as kids get older and for teens, then we use dorsal lithotomy, as we would in adults.
00:13:35.039 --> 00:13:45.519
But there are pediatric-sized stirrups, which are really helpful because proper sizing and proper support for leg positioning is going to help protect against nerve injury.
00:13:45.519 --> 00:13:52.559
We should check the arms so that we can optimize surgeon ergonomics and patient safety and trendelingberg positioning.
00:13:52.559 --> 00:13:56.720
But when it comes to positioning, it's always about protecting against nerve injury.
00:13:56.720 --> 00:14:03.679
So femoral nerve injury, perineal, ulnar injuries are all possible if positioning isn't super careful.
00:14:03.679 --> 00:14:14.879
And so we need to focus on proper alignment, padding, positioning similarly to how you would position an adult patient for laparoscopic pelvic surgery.
00:14:15.279 --> 00:14:16.080
Absolutely.
00:14:16.080 --> 00:14:31.279
And then we all know that we must be very careful not to lean on our patients because we can cause injuries by just leaning on a leg, leaning on an arm, leaning on the upper abdomen above our laparoscopic camera.
00:14:31.279 --> 00:14:39.360
So once we are ready to introduce our instruments, what are the preferred sites and what pressures do we use?
00:14:39.679 --> 00:14:49.840
When it comes to entry, we prefer umbolical entry in kids and teens as it uses that confluence of the abdominal wall layers to provide the thinnest entry point.
00:14:49.840 --> 00:14:55.840
Ferris entry, direct entry, open laparoscopic entry have all been described.
00:14:55.840 --> 00:15:02.799
And we need to remember, as we touched on earlier, that the integrity of the abdominal wall tissues changes with age.
00:15:02.799 --> 00:15:06.320
And our abdominal entry is going to be impacted by that.
00:15:06.320 --> 00:15:19.679
Newborns and infants have a lot of laxity and pliability in their tissues, but the fascia tension and strength increases with age until adolescence, young adulthood, and that can definitely impact our entry technique.
00:15:20.000 --> 00:15:22.080
What's your preferred entry method?
00:15:22.320 --> 00:15:23.679
I prefer a varus entry.
00:15:23.840 --> 00:15:25.919
That's what I've done all my career, too.
00:15:26.639 --> 00:15:32.399
It's very important also to remember the distance from the abdominal wall to the large abdominal vessels.
00:15:32.399 --> 00:15:38.879
That distance is shorter in pediatric and adolescent patients, and so it requires a lot of care.
00:15:39.200 --> 00:15:49.759
And then in the if we're going to place accessory ports, we want to avoid neurovascular damage to the vessels and nerves that are traveling down in the rectus sheath.
00:15:49.759 --> 00:16:01.840
So the guidelines are to place accessory ports superior to the anterior-superior iliac spines greater than six centimeters from the abdominal midline.
00:16:02.240 --> 00:16:12.799
And I'll mention here too that we've been talking mostly about straight stick laparoscopy, but there can be a role for using robotic surgery for patients with more complex anatomy as well.
00:16:12.799 --> 00:16:24.320
It's definitely a developing space in terms of best use cases for pediatric and adolescent populations, thinking about advantages, challenges, and safety.
00:16:24.639 --> 00:16:29.600
That'll be an interesting development in pediatric and adolescent kinecologic surgery.
00:16:30.000 --> 00:16:42.799
We mentioned insufflation pressures before, and I want to go back to that because we definitely think about lower insuflation pressures, and we can kind of categorize it a little bit based on age group.
00:16:42.799 --> 00:16:49.600
So typically for infants, we would use distension pressures from about six to eight millimeters of mercury.
00:16:49.600 --> 00:16:55.679
Then when we think about younger children, we think about that eight to ten millimeters of mercury range.
00:16:55.679 --> 00:17:01.679
And then for older children and adolescents, we're thinking between 10 and 15 millimeters of mercury.
00:17:01.679 --> 00:17:08.480
Another adjustment that we may need to make in equipment settings is electrocottery wattage.
00:17:08.480 --> 00:17:14.000
And so that should also be adjusted for younger patients and kept at the lowest effective power.
00:17:14.240 --> 00:17:15.759
What about Trendellenberg?
00:17:16.079 --> 00:17:30.400
In my experience, it's been similar to working with an adult patient in that we want to optimize visualization while working closely with our anesthesiologist to make sure that ventilation is still going well.
00:17:30.400 --> 00:17:32.640
Just thinking about fascial closure.
00:17:32.640 --> 00:17:40.000
And so, of course, in any age group, anything that's 10 millimeters or larger will definitely be closing the fascia.
00:17:40.000 --> 00:17:50.559
But we do consider for younger patients closing even smaller, even five millimeter ports because herniations have been reported in that younger age group.
00:17:50.880 --> 00:17:57.440
So now we have operated on our patient and everything has gone great.
00:17:57.440 --> 00:18:01.279
And now we're going to talk about postoperative pain management.
00:18:01.279 --> 00:18:19.119
Historically, children and teenagers have been undertreated for their postoperative pain on the assumption that they may not be experiencing as much pain as an adult, or they cannot communicate about the pain that they are experiencing.
00:18:19.119 --> 00:18:33.279
So there are age-appropriate scales that are used in hospitals, including the CHEOP scale, the FLAC scale, F L A C C, and the FACES scale, F A C E S.
00:18:33.279 --> 00:18:41.680
It is important to evaluate patients at any age, so don't assume we can't evaluate our nonverbal patients.
00:18:41.680 --> 00:18:54.160
And it's important to treat their pain, to prevent postoperative pain sensitization, and to help with recovery in terms of their GI motility, their mobility, and so forth.
00:18:54.160 --> 00:19:00.400
Often pain control is left to the parents once the child is leaving the hospital.
00:19:00.400 --> 00:19:14.559
So we really have to be cognizant of giving them very sound guidance on pain treatment and also whether to use scheduled pain treatment or symptomatic pain treatment.
00:19:14.559 --> 00:19:22.160
We use a combination of acetomenophen, NSAIDs, limited narcotics, and blocks when appropriate.
00:19:22.480 --> 00:19:30.880
Having a regional anesthesia involved can be really helpful for something like a transverse abdominus plane block or a quadratus limborum block.
00:19:30.880 --> 00:19:35.920
And depending on the type of surgery, that's definitely something we talk about with our anesthesia teams.
00:19:36.240 --> 00:19:41.200
And talk about the development of Aeris for pediatric and adolescent patients.
00:19:41.839 --> 00:19:51.920
Aeris protocols have been developed over the years in adult gynecology, pediatric urology, pediatric surgery, and they're now becoming more commonplace in pediatric gynecology as well.
00:19:51.920 --> 00:20:02.400
So these are guidelines that focus on early feeding, mobilization, and non-narcotic pain management to support an earlier return to school and other activities.
00:20:02.400 --> 00:20:15.920
As institutions come on board, it is great to see the teamwork that comes with having an ARS guideline to follow that really gets our patients back on their feet and recovering better and sooner.
00:20:16.319 --> 00:20:25.839
I have found in throughout my surgical career that teens tend to bounce back really quickly, sometimes too quickly.
00:20:25.839 --> 00:20:32.480
Like I had a teenager who I told her, you know, not to go to dance class for at least a week.
00:20:32.480 --> 00:20:38.559
And she comes back for her post-op visit and she went back to dance class two days after her surgery.
00:20:38.559 --> 00:20:48.079
So we, you know, they do recover quickly, but we have to make sure we're putting proper pain management and other guidelines in place.
00:20:48.400 --> 00:20:54.720
I like to tell kids to listen to their body, and that gives them a little bit of agency in the process.
00:20:54.720 --> 00:21:11.759
But you're right, that adding that counseling around why we're limiting activity, why it's important to take things a little bit easy as you're recovering from surgery can be helpful in involving them and getting their buy-in for this recovery process.
00:21:11.759 --> 00:21:18.720
I think another thing that's helpful on that note is even considering when we plan an elective surgery.
00:21:18.720 --> 00:21:31.359
You know, if we have the ability to plan our surgery ahead of time, then it is helpful to schedule it in a way that minimizes disruption to school, to home, social activities for the patient.
00:21:31.359 --> 00:21:48.559
You know, I've booked surgeries around volleyball season, for example, something where I'm not going to be impacting that important social physical development that's happening in those early years, but also addresses obviously what needs to be done surgically for the patient.
00:21:48.799 --> 00:21:50.240
Yeah, absolutely.
00:21:50.240 --> 00:21:58.960
And another concern that people have when they're having laparoscopic surgery is will there be any impact on their fertility?
00:21:58.960 --> 00:22:15.359
So it is certainly very important postoperatively that we review thoroughly what we have done and to address whether there may or may not be any impact on fertility, both with the patient and with their parents.
00:22:15.359 --> 00:22:18.079
Okay, so some takeaway points.
00:22:18.400 --> 00:22:21.200
Pediatric analysis and patients are not just small adults.
00:22:21.200 --> 00:22:24.480
So anatomic and physiologic differences really matter.
00:22:24.480 --> 00:22:33.200
And careful planning of those pre-operative, intra-operative, postoperative considerations does help to minimize complications and improve recovery.
00:22:33.200 --> 00:22:50.960
But I think if I can say just one key takeaway and key thought is that it's always to center the patient, include the patient in decision making, use age-appropriate tools, and think about their daily life and their recovery and place the surgery in that context for them.
00:22:51.200 --> 00:22:58.559
There are some videos covering positioning, prep, abdominal entry on YouTube for people who want to look them up.
00:22:58.559 --> 00:23:05.839
So this concludes our podcast on laparoscopic considerations for pediatric and adolescent patients.
