ABOUT THIS EPISODE
A 105-degree fever, weeks of rapid weight loss, and no clear answers sent Dr. John Maltby down a path he never expected. One chiropractic visit changed everything and that moment eventually led him to 48 years in practice and a new role as Secretary General of the World Federation of Chiropractic (WFC). We sit down with John to hear the full story, from a teenage health crisis to building a long-term practice in a small farming community, and why he believes chiropractic’s impact grows when we stay focused on results that matter to real people.
We also get specific about what the WFC actually does and why its relationship with the World Health Organization (WHO) is such a big deal for global health. John breaks down how the WFC represents chiropractic to the WHO, helps open doors with governments and health leaders, and supports a simple vision: access to chiropractic care for all people, of all ages, in all nations. Along the way, we talk about the profession’s biggest obstacle: internal division that confuses the message when the world is asking one question, “How can you help our people be healthy?”
From low back pain as a leading cause of disability worldwide to the everyday goal of staying mobile as we age, we connect chiropractic, rehabilitation, and quality of life. John also shares why the adjustment remains the profession’s unique core, plus an exciting look at sports chiropractic momentum and how close chiropractic is to being part of every Olympic team. Subscribe, share this with a colleague or patient, and leave a review, what do you think chiropractic should be known for globally?

00:00:07.200 --> 00:00:07.919
Hi, I'm Dr.
00:00:08.080 --> 00:00:11.599
Arlan Four, the chairman and founder of Activator Methods International.
00:00:11.839 --> 00:00:17.120
And uh we're bringing you today Activate Your Practice, which is our podcast.
00:00:17.199 --> 00:00:26.800
And I'm happy to report to you that our 2025 numbers, we had over 2,400,000 people that downloaded this podcast.
00:00:26.960 --> 00:00:29.199
So we are reaching more than chiropractors.
00:00:29.519 --> 00:00:33.679
And today I have the distinct honor to have a very important person.
00:00:33.920 --> 00:00:38.640
He's the new Secretary General of the World Federation of Chiropractic, Dr.
00:00:38.719 --> 00:00:39.679
John Maltby.
00:00:40.000 --> 00:00:41.280
John, welcome to the podcast.
00:00:41.679 --> 00:00:43.439
Good to be here, Ron, and thanks for inviting me.
00:00:43.840 --> 00:00:56.479
Well, John had a bit of a I would say a surprise this year that uh he got shoved into the position of the Secretary General when someone else uh was resigning.
00:00:56.719 --> 00:01:04.959
And uh I've known John for a long time, and he always tells me he's the son of a plumber, and uh he's just uh a smart son of the plumber.
00:01:05.120 --> 00:01:07.439
And uh so welcome to the podcast, John.
00:01:07.760 --> 00:01:08.319
Good to be here.
00:01:08.480 --> 00:01:09.040
Good to be here.
00:01:09.280 --> 00:01:13.599
And uh give us a little history on uh what was your chiropractic story?
00:01:13.680 --> 00:01:14.959
In other words, how'd you start?
00:01:15.359 --> 00:01:18.560
Well, my chiropractic story probably started in high school.
00:01:18.799 --> 00:01:21.760
My sophomore year, I was a pretty good swimmer, doing really well.
00:01:22.000 --> 00:01:42.000
My junior year, I dropped sick with 105 temperature, bad throat, lost 25 pounds in two weeks, uh, went the whole medical model, thought I got well and didn't get well, ended up at the Air Force Academy, got sick again, came home and uh was lying in bed, and one of my friends, a guy named Dave Cobble, came by and said, uh, you need to see a chiropractor.
00:01:42.400 --> 00:01:43.920
And I go, What's a chiropractor?
00:01:44.159 --> 00:01:44.799
Never heard of it.
00:01:44.959 --> 00:01:48.640
So he actually took me to a lay lecture on chiropractic and listened to the guy's story.
00:01:48.799 --> 00:01:55.519
The next day I went to a relatively new chiropractor in Westminster, that uh Southern California, a guy named Mark Worthlund.
00:01:55.680 --> 00:02:00.000
He took X-rays, adjusted my neck and my full spine.
00:02:00.159 --> 00:02:04.319
My fever broke immediately, and I never had that problem ever again.
00:02:04.480 --> 00:02:08.400
I had my first successful season since my sophomore year of high school.
00:02:08.639 --> 00:02:12.319
And uh yeah, so so it magically changed my life.
00:02:12.479 --> 00:02:16.159
Uh my intent was always to go become a lawyer and go into politics.
00:02:16.479 --> 00:02:20.240
Uh stuff happened around the early 70s that really discouraged me with that.
00:02:20.400 --> 00:02:22.879
So I said, I'm not gonna do anything to do with the politics.
00:02:23.120 --> 00:02:24.560
Why don't I be a chiropractor?
00:02:24.719 --> 00:02:26.319
Let's touch one person at a time.
00:02:27.039 --> 00:02:31.439
So I took$600, jumped in my Rambler, and drove to Davenport, Iowa.
00:02:31.759 --> 00:02:33.120
And uh the rest's been history.
00:02:33.199 --> 00:02:34.240
I've been in practice next week.
00:02:34.319 --> 00:02:40.319
We'll be 48 years in practice in Blythe, California, a small farming community on the Colorado River.
00:02:40.560 --> 00:02:43.840
And um Debbie and I just celebrated 50 years of marriage.
00:02:43.919 --> 00:02:44.479
I got two kids.
00:02:44.560 --> 00:02:47.840
My son's a chiropractor and uh 11 grandkids.
00:02:48.080 --> 00:02:52.080
Um what got you involved then in chiropractic politics?
00:02:52.159 --> 00:02:54.240
Because you said you didn't want to get into politics.
00:02:54.639 --> 00:03:01.520
Well, it it started in California, the second largest country of chiropractors in the world at that time with 14,000.
00:03:01.599 --> 00:03:06.000
And I got calls said, Hey, would you be interested and get it on the ICA California board?
00:03:06.080 --> 00:03:07.280
I go, Yeah, I'll show up.
00:03:07.439 --> 00:03:10.879
Well, then it kind of grew, some stuff happened about a year into that.
00:03:11.120 --> 00:03:13.759
Uh, they needed a president, and they go, Will you do that?
00:03:13.840 --> 00:03:14.479
And I go, yeah.
00:03:14.560 --> 00:03:17.599
Well then, same thing, kind of like with ICA.
00:03:18.000 --> 00:03:27.039
Uh, there was an opening, there was a battle going on anyway, so I became Western Regional Director, and then the next year I became president because of a very similar situation.
00:03:27.280 --> 00:03:33.919
And then about 10 years ago, Jerry Klum called me and asked me to take his place on the WFC board.
00:03:34.719 --> 00:03:35.840
And I said, Okay.
00:03:36.560 --> 00:03:45.599
And I was planning on stepping off and stepping back and getting away this year, and stuff happened, and I got a call, said, You need to do this job.
00:03:45.840 --> 00:03:47.599
And my wife supported me on it.
00:03:47.840 --> 00:03:48.960
So here I am.
00:03:49.360 --> 00:03:53.840
Well, discuss the current pos you know, your WFC Secretary General.
00:03:53.919 --> 00:03:55.039
It's a big term, you know.
00:03:55.199 --> 00:03:56.639
Tell me what does it involve?
00:03:57.120 --> 00:03:59.680
Well, the WFC, first of all, is 40 years old.
00:03:59.919 --> 00:04:09.599
Um, it was an organization that was started for one thing to bring representative representation of chiropractic from around the world and develop direct relations with the WHO.
00:04:09.919 --> 00:04:22.160
So as a result, the WHO is the uh pardon me, the WFC is the only chiropractic association in the world that has what uh non-state actor affiliation with the WHO.
00:04:22.560 --> 00:04:25.519
So that we actually go, we have standing, we serve on committees.
00:04:25.680 --> 00:04:31.040
I think the big thing I want people to understand is we represent chiropractic to the WHO.
00:04:31.519 --> 00:04:34.639
We are not here to represent the WHO chiropractic.
00:04:35.519 --> 00:04:39.120
As of right now, we have 95 countries that are involved.
00:04:39.439 --> 00:04:56.160
We have seven regions, uh we do our assembly, our educational conferences, but the main thing is fulfilling the vision of the WFC, which is very simply all people, all ages, all nations have access to chiropractic care.
00:04:56.800 --> 00:05:01.120
Um what's the biggest obstacle to chiropractic moving forward?
00:05:01.519 --> 00:05:05.360
Yeah, well, I you know, I usually get in trouble for this, but I'm pretty used to getting in trouble.
00:05:05.600 --> 00:05:07.920
I think it's the chiropractic profession sometimes.
00:05:08.399 --> 00:05:13.600
I think it's the naysayers out there, the ones, the guys that want to create division within the profession.
00:05:13.920 --> 00:05:16.720
Instead of those of us just saying, you know, enough, stop it.
00:05:16.959 --> 00:05:22.399
Let's come together, let's figure out the things we agree on, and let's advance chiropractic.
00:05:22.879 --> 00:05:32.240
Because when I go to different countries and I go to Rwanda, I spend a week in Rwanda meeting with, you know, top top cabinet members and we and presenting chiropractic.
00:05:32.560 --> 00:05:36.720
And if we would have gone in with the different arguments that are going on, they would have no idea what we're talking about.
00:05:37.120 --> 00:05:38.000
They want to know one thing.
00:05:38.160 --> 00:05:39.920
How can you help our people be healthy?
00:05:40.399 --> 00:05:40.959
That's it.
00:05:41.360 --> 00:05:45.120
You know, and that we are the alternative to drugs and surgery.
00:05:45.680 --> 00:05:47.600
That we are not physical therapists.
00:05:48.079 --> 00:05:52.160
We do something that is unique and that's the adjustment.
00:05:52.720 --> 00:05:52.959
Okay?
00:05:53.519 --> 00:05:54.480
Can we do the other stuff?
00:05:54.639 --> 00:05:55.040
Of course.
00:05:55.360 --> 00:06:00.560
But but our mission is to open up doors for chiropractic around the world.
00:06:01.040 --> 00:06:09.360
And because of our affiliation, WFC's affiliation with the uh WHO, that opens those doors.
00:06:09.680 --> 00:06:12.160
And that's what the naysayers don't understand.
00:06:12.399 --> 00:06:14.879
It's about getting the doors open.
00:06:15.120 --> 00:06:20.000
So I've always said, you know, for some reason we've had a we we we kind of want to fight among ourselves.
00:06:20.160 --> 00:06:20.959
What's the old saying?
00:06:21.040 --> 00:06:22.959
We circle the wagons and shoot to the middle.
00:06:23.600 --> 00:06:27.519
Well, you know, uh, this has been a several years ago, but uh Dr.
00:06:27.600 --> 00:06:35.519
George Goodman from Logan invited my wife and I to come to Switzerland and to come be a delegate to the WHO.
00:06:36.160 --> 00:06:41.680
And uh as I looked in the auditorium that day, it was kind of divided in half.
00:06:41.839 --> 00:06:43.439
I just my overview.
00:06:43.680 --> 00:06:49.920
Half of it was infections in the in the world, the countries that were not up, you know, third world countries.
00:06:50.160 --> 00:07:02.480
The other half was all of the musculoskeletal problems that plagued the United States and the people that were suffering from arthritis and from all the different things that were were structural problems.
00:07:02.639 --> 00:07:06.160
And I thought, oh my gosh, chiropractic's got a big role to play.
00:07:07.279 --> 00:07:11.519
And I just I I thought we we need to have we needed to let them know what we do.
00:07:11.920 --> 00:07:12.160
Right.
00:07:12.639 --> 00:07:14.240
And again, that's the point.
00:07:14.319 --> 00:07:18.319
If I in fact, when I go to Rwanda, when we sit down, a lot of people understand this.
00:07:18.399 --> 00:07:22.000
The number one disability in the world is low back pain.
00:07:22.480 --> 00:07:26.160
And I always like to say in in the United States, what does that mean?
00:07:26.399 --> 00:07:38.480
If you're disabled because of low back pain, you got state disability, you got work comp, you got insurance, you got all these different access to the what does that mean in a third world country if you're disabled because of low back pain?
00:07:38.720 --> 00:07:40.480
It means your children die.
00:07:41.279 --> 00:07:44.240
You can't work to feed your children.
00:07:44.560 --> 00:07:46.639
I mean, so it's a whole lot bigger picture.
00:07:47.120 --> 00:07:56.560
Well, in the United States, uh as I the older I get, the more I see people, you know, that lose their ambulation or ability to get around.
00:07:56.720 --> 00:08:01.120
And that's one of the staying ambulatory is one of the biggest things that I can see.
00:08:01.279 --> 00:08:04.480
And that's where I think you'll agree, chiropractic comes in in great form.
00:08:04.879 --> 00:08:06.160
Oh, it's all about motion.
00:08:06.319 --> 00:08:07.839
You know, it's all it's all about structure.
00:08:08.000 --> 00:08:09.839
I mean, what's the basic law of physics?
00:08:10.160 --> 00:08:11.519
Form equals function.
00:08:11.680 --> 00:08:12.720
What do we deal with?
00:08:12.959 --> 00:08:14.160
We deal with form.
00:08:14.399 --> 00:08:19.680
We deal with, you know, you know, getting the spine in a normal position that it was designed to be in.
00:08:19.920 --> 00:08:24.079
Because if you deviate from that normal, then you you you change the function.
00:08:24.399 --> 00:08:29.680
And so we as chiropractors need to make sure that we maintain what makes us unique.
00:08:29.920 --> 00:08:31.360
And that's what makes us unique.
00:08:31.519 --> 00:08:33.279
We adjust, we're correcting the spine.
00:08:33.440 --> 00:08:34.240
Can you do the other stuff?
00:08:34.399 --> 00:08:35.120
Yeah, I don't care.
00:08:35.200 --> 00:08:37.519
But everybody else does the uh the other stuff too.
00:08:37.759 --> 00:08:39.360
What's unique to us?
00:08:39.840 --> 00:08:41.440
The correction of the spine.
00:08:42.159 --> 00:08:45.200
You know, I practiced for quite a few years.
00:08:45.440 --> 00:08:50.720
And I Monday, it was always my big day, and I had a young student doctor follow me around one day.
00:08:50.879 --> 00:08:52.159
He said, Why do these people all coming?
00:08:52.399 --> 00:08:54.320
I said, Because they want to stay mobile.
00:08:54.559 --> 00:08:57.039
And a lot of more geriatrics in that day.
00:08:57.279 --> 00:09:02.240
And their biggest reason for coming to me for continued care was to keep their mobility.
00:09:02.480 --> 00:09:02.720
Right.
00:09:03.039 --> 00:09:05.120
And so I think that's something that we can get out of.
00:09:05.600 --> 00:09:06.320
Oh, absolutely.
00:09:06.559 --> 00:09:07.120
Absolutely.
00:09:07.279 --> 00:09:10.639
I mean, the big word out there now is rehabilitation, okay?
00:09:10.799 --> 00:09:16.080
I mean, there's big committees and WHO is doing a lot of stuff under the rehabilitation.
00:09:16.320 --> 00:09:17.039
Well, guess what?
00:09:17.200 --> 00:09:19.519
We we we do that's what we do.
00:09:19.759 --> 00:09:24.399
I mean, we rehabilitate the body by specifically dealing with spinal structure.
00:09:24.559 --> 00:09:25.600
Again, can we do other stuff?
00:09:25.759 --> 00:09:26.559
Of course you can.
00:09:26.720 --> 00:09:31.840
But chiropractic is unique and it's making that message clear on a global scale.
00:09:32.240 --> 00:09:40.799
And I think for all the patients that are listening here, that if you can keep mobile and keep from being immobile, that's that's to what your goal is.
00:09:41.120 --> 00:09:41.679
All about motion.
00:09:41.759 --> 00:09:42.000
Absolutely.
00:09:42.320 --> 00:09:43.759
Keeping your, you know, all about motion.
00:09:44.080 --> 00:09:46.159
Somebody said to me, how do you get around as good as you do?
00:09:46.240 --> 00:09:50.399
And I said, I've got two schnauzers, you know, that I have to walk daily.
00:09:50.639 --> 00:09:56.080
And uh that's why they say pet owners are more, you know, healthy because they're out walking their dogs.
00:09:56.320 --> 00:09:57.759
And remember, I'm still in practice.
00:09:57.840 --> 00:09:59.120
I'm still seeing patients, right?
00:09:59.279 --> 00:10:01.600
And this time of year, you know who I'm seeing.
00:10:01.679 --> 00:10:03.120
I'm seeing the snowbirds.
00:10:03.279 --> 00:10:04.320
I mean, it's not uncommon.
00:10:04.399 --> 00:10:09.519
The average aged patient on visit in in my office any day is 70 to 80 years old.
00:10:09.759 --> 00:10:12.799
Okay, and the whole idea is to keep them moving.
00:10:13.039 --> 00:10:14.960
We live for motion.
00:10:15.279 --> 00:10:17.279
If you don't move, you die.
00:10:17.679 --> 00:10:21.919
So, what do you see as the biggest opportunity moving forward?
00:10:22.320 --> 00:10:25.600
I think the biggest opportunity, uh and and this is a great weekend.
00:10:25.679 --> 00:10:33.840
Here we are at Parker Vegas, and to see the enthusiasm that people now are starting to look more globally.
00:10:34.320 --> 00:10:37.440
Because of electricity, the world's got smaller.
00:10:37.840 --> 00:10:40.639
We have the ability to see what's going on around the world.
00:10:40.720 --> 00:10:53.200
And we and when we tell people about what's happening in chiropractic in third world countries, in Latin America, in Mexico, and we started to see, I met with Fix yesterday, the sports chiropractic group.
00:10:53.360 --> 00:11:01.200
I mean, we're that close to having chiropractors officially part of every Olympic team.
00:11:01.759 --> 00:11:02.639
We're that close.
00:11:03.039 --> 00:11:06.480
In fact, it was an interesting they shared with us statistically, they kept the numbers.
00:11:07.360 --> 00:11:14.240
The number one sought-after service by athletes is dentistry.
00:11:15.519 --> 00:11:16.159
That's crazy.
00:11:16.480 --> 00:11:16.960
Dentistry.
00:11:17.200 --> 00:11:18.240
Yeah, they get free dentist care.
00:11:18.320 --> 00:11:24.639
But but but but the thing is interesting that number two, 34% is chiropractic.
00:11:25.279 --> 00:11:27.120
Because they know what they have to do as athletes.
00:11:27.440 --> 00:11:32.559
I mean, and and and nothing against the other ones, but the other ones out there are like half of what we are.
00:11:32.879 --> 00:11:37.360
So they actually seek if chiropractors are available, they seek them.
00:11:37.519 --> 00:11:38.320
They want them.
00:11:38.639 --> 00:11:42.159
Between the World Games and the Olympics, I mean, it's really, really exciting.
00:11:42.320 --> 00:11:44.480
So it's that type of exposure.
00:11:44.799 --> 00:11:51.600
And that these some of these athletes who don't have chiropractic in the country can come to these events and find out about chiropractic.
00:11:51.759 --> 00:11:52.320
Guess what?
00:11:52.480 --> 00:11:55.120
They're doing everything they can to get it back in the country.
00:11:55.279 --> 00:12:03.840
So, because of that, because of speed of light information, because of the things that we're doing within the WFC to communicate that globally.
00:12:04.399 --> 00:12:05.840
I think it's really exciting.
00:12:06.080 --> 00:12:07.200
I think it's really exciting.
00:12:07.600 --> 00:12:13.519
Well, I just want to wish you good luck in your new venture here as the WFC Secretary General.
00:12:13.600 --> 00:12:16.799
And I know that you're going to be traveling a lot and seeing a lot of different things.
00:12:16.879 --> 00:12:24.799
And perhaps after you've been around for a year and can see more things, then we'll put you back on the podcast so that we can let everybody know what's going on.
00:12:25.120 --> 00:12:25.440
Perfect.
00:12:25.519 --> 00:12:26.159
Thank you, Ron.
00:12:26.480 --> 00:12:27.120
You're very welcome.
00:12:27.360 --> 00:12:27.519
Thank you.
00:12:27.679 --> 00:12:27.840
