درباره این اپیزود
You've asked a doctor about a supplement, an herb, or a therapy like hyperbaric oxygen or ozone, and gotten a flat "that doesn't work" in return, with no real explanation and no room for follow up questions.
Why this episode matters:
Dr. Su has watched this happen to his own patients for years. In this episode he names exactly what's going on and gives it a term, ontological humility, the simple idea that we don't know what we don't know. He applies it to both sides of the exam room, and by the end you'll understand not just why doctors dismiss what they don't know, but why patients need to ask the same honest questions about the herbs and supplements they trust.
What you will learn:
Why conventional doctors often say "it doesn't work" when the honest answer is "I don't know enough to say"
Why "natural" doesn't automatically mean "safe," and what's actually missing from the research on herbs and supplements
How to think about the real risk of what you're taking, instead of assuming risk is zero because it came from a health food store
Key themes:
Ontological humility as a framework for honest practice
The time crunch behind conventional medicine's dismissiveness
The unregulated gray zone of supplement and herbal research
Dr. Su doesn't let either side off the hook here. He's direct that dismissing a therapy just because you're unfamiliar with it is unfair and unjust, and he's equally direct that assuming a supplement is safe just because it's not a pharmaceutical is its own kind of unearned confidence. His answer isn't to pick a side, it's to be honest about what's actually known and unknown, and to weigh real risk before making a decision.

Let's get real and get results.
00:00:41.600 --> 00:00:51.280
Hey friends, so if you've ever had an experience where you wish your conventional practitioner was more open-minded about non-conventional therapies, you're gonna like this one.
00:00:51.439 --> 00:00:53.920
Or you might at least like the first half of this one.
00:00:54.079 --> 00:00:54.479
All right.
00:00:54.640 --> 00:00:56.560
So let me introduce a term for you.
00:00:56.719 --> 00:00:58.960
It's called ontological humility.
00:00:59.119 --> 00:01:02.079
And it means we don't know what we don't know.
00:01:02.320 --> 00:01:02.719
All right?
00:01:02.880 --> 00:01:04.079
That's the humility part.
00:01:04.239 --> 00:01:10.319
And so what happens a lot in conventional medicine is I'd like to say a lot of it's just because of the time crunch, all right?
00:01:10.480 --> 00:01:16.640
The the lesser insufficient time for visits and more meaningful discussion.
00:01:16.879 --> 00:01:38.239
When patients ask conventional practitioners about therapies, supplements, herbs, procedures, let's say like hyperbaric oxygen chamber, even or ozone therapy, et cetera, when a practitioner is not familiar with it and they're asked to give an opinion on it, it's pretty common the answer is it doesn't work, or it's not good for you, or bad things can happen.
00:01:38.400 --> 00:01:38.879
All right.
00:01:39.040 --> 00:01:41.680
So it's just the bottom line is it's not right.
00:01:41.840 --> 00:01:44.480
It's unfair, it's unjust, all right?
00:01:44.719 --> 00:01:48.560
The real answer is I don't know enough about it to give you an answer.
00:01:48.640 --> 00:02:03.040
And there are conventional practitioners out there who will be honest and say that, but it's really common, and I know this from firsthand experience with my patients, when they see a a a former primary care doctor or a specialist, commonly it's a little bit of a cop-out, to be honest with you, in my opinion.
00:02:03.120 --> 00:02:04.079
It's it's a liability.
00:02:04.159 --> 00:02:11.199
Sometimes it's a liability response, but a lot of times I think it's just the easy way to shut down that conversation, if you will, right?
00:02:11.280 --> 00:02:14.639
And it's not to be, I believe they're not trying to be mean to the patients.
00:02:14.800 --> 00:02:19.919
They just don't have enough time or don't really want to engage in in a conversation.
00:02:20.000 --> 00:02:23.520
And it's a lot of work to keep learning new stuff over and over, right?
00:02:23.680 --> 00:02:29.599
So the practitioners who they they they're not inclined to spend the time to figure to learn something new.
00:02:29.680 --> 00:02:30.080
All right.
00:02:30.319 --> 00:02:37.280
Again, not a judgment, just saying that's the my strong observation and and conviction about how the conventional medicine works.
00:02:37.439 --> 00:02:39.120
Now, here's the flip side of that.
00:02:39.280 --> 00:02:56.639
A lot of patients, a lot of us, abide by the presumption that when something is non-pharmaceutical as a treatment, it's a supplement, it's an herb, that it's more natural and therefore better for us, healthy for us, maybe safer for us, okay, or all the above.
00:02:56.800 --> 00:03:02.560
And it's pretty easy to make that argument, especially if we're talking about bioidentical therapies, all right?
00:03:02.719 --> 00:03:19.680
Not just biodental hormones, but I mean vitamins, biodentical, other non-vitamin supplements, et cetera, putting aside, compartmentalizing the topic of the integrity of the supplement companies that make these products, which don't have a lot of regulations, don't have more meaningful regulation, all right.
00:03:19.840 --> 00:03:22.000
Putting that aside, I acknowledge that piece.
00:03:22.159 --> 00:03:29.439
The fact of the matter is a lot of non-pharmaceutical therapies, let's say especially the herbals, again, not passing judgment on them either, okay?
00:03:29.599 --> 00:03:34.319
So if you're a naturopath or uh you're into herbal therapies, I'm it's it's not a shade.
00:03:34.400 --> 00:03:38.719
It's just we only know what we know from rather limited research, okay?
00:03:38.879 --> 00:03:43.520
Supplement companies, herbal companies, they don't have the money to put in the research.
00:03:43.680 --> 00:03:50.159
They don't have the money to put in all the studies to pass muster for FDA approval level of studies.
00:03:50.319 --> 00:03:54.319
My dad, in you know, in another in other content, I'll make more comment about this.
00:03:54.400 --> 00:04:01.039
But my dad actually was a researcher for a major pharmaceutical company, and that's why we grew up in the city we l we grew up in.
00:04:01.199 --> 00:04:02.960
And uh he was an all-comer, all right.
00:04:03.039 --> 00:04:06.000
He was open-minded to both pharmaceutical and non-pharmaceutical.
00:04:06.400 --> 00:04:09.680
He tended to shy away from pharmaceuticals of certain types, all right?
00:04:09.919 --> 00:04:14.240
But that's the kind of balance and the origin that I come from, all right, the fabric I come from.
00:04:14.479 --> 00:04:26.639
But he had due respect for the uh at least the process of what require, what was required to pass a medication by FDA approval to get full presentation to the market, if you will, to the public for a pharmaceutical medication.
00:04:26.720 --> 00:04:28.319
It's a lot of money.
00:04:28.560 --> 00:04:29.040
All right.
00:04:29.199 --> 00:04:32.720
And the farms and the and the supplement and herbal companies don't have that kind of money.
00:04:32.959 --> 00:04:58.079
So when we don't have the data, when we don't have more information about how many of the supplements and herbs we use might have herb-to-herb interaction, herb-to-medication interaction, supplement to medication interaction, or whether any of the how much or whether various supplements and herbs might actually cause us some side effects, or in some patients be more problematic than other patients at the least, okay?
00:04:58.240 --> 00:04:59.199
We just don't know.
00:04:59.360 --> 00:05:01.680
So the bottom line is let's call a spade a spade.
00:05:01.759 --> 00:05:02.319
Let's be fair.
00:05:02.480 --> 00:05:04.000
Let's be fair on both sides of the coin.
00:05:04.160 --> 00:05:16.000
Yes, in the conventional medicine world, it is unfair for us as practitioners to say something is no good, bad, um, judge it when we just don't really actually know.
00:05:16.399 --> 00:05:16.879
All right.
00:05:17.040 --> 00:05:21.199
Similarly, let's be fair when it comes to supplements on herbs.
00:05:21.360 --> 00:05:25.759
We we also need to be open-minded that there may be things that we don't know about them either.
00:05:25.920 --> 00:05:26.240
All right.
00:05:26.319 --> 00:05:28.560
And that's the world we live in in functional medicine.
00:05:28.800 --> 00:05:35.279
The the the gray zone, the less known, we have lots of tools and lots of potential, lots of hope.
00:05:35.360 --> 00:05:38.879
But the flip side of the coin is we're wrestling with a lot of unknowns.
00:05:39.120 --> 00:05:44.079
That then it ultimately comes down to what's the risk we're taking, what's the potential risk, all right?
00:05:44.240 --> 00:05:51.040
The risk of taking fish oil, of taking folic acid, magnesium, et cetera, et cetera, minimal to none, arguably, okay?
00:05:51.199 --> 00:05:51.839
So there you have it.
00:05:52.000 --> 00:05:58.160
A little more piece of the reality of functional medicine as I see it, and the intersect between functional and conventional medicine.
00:05:58.319 --> 00:06:00.720
Keeping it real, keeping it real for me.
00:06:00.879 --> 00:06:04.079
You're welcome to I invite you to keep me in check and keeping it real.
00:06:04.240 --> 00:06:19.680
Keep up the good work that you're doing in taking care of yourself, making the decisions, the best decisions you can for you and your family, knowing that um some of these decisions are not the most easiest, they're not the easiest to make, as we're still wanting for more information to make even better decisions if we could.
00:06:20.399 --> 00:06:22.800
So fresh, just the way you like to.
