What happens after a senior leaves the hospital? For many families, this is one of the most confusing—and most important—times in the healthcare journey. The decisions made during the first few days after discharge can have a lasting impact on a senior's recovery, independence, and quality of life.
On this episode of Solving Life's Next Chapter, host Lydia Wietsma, Certified Probate & Senior Real Estate Specialist, welcomes Kerri Cummiskey of Alumus Healthcare for an important conversation about Understanding Transitional Care and how families can better prepare for life after a hospital stay.
One of the biggest mistakes families make is waiting until a medical crisis occurs before learning about their options. Lydia and Kerri discuss why it's essential to plan ahead by understanding what your health insurance covers, what services may require prior authorization, what out-of-pocket expenses you may face, and the differences between skilled nursing, rehabilitation, home health, assisted living, and long-term care. Knowing these answers before they're needed can reduce stress, prevent costly surprises, and help families make confident decisions during an already emotional time.
During this educational episode, Lydia and Kerri cover:
- What transitional care is and why it is critical to a successful recovery.
- The different levels of post-acute care, including skilled nursing, rehabilitation, home health, outpatient therapy, and hospice.
- Why understanding your Medicare, Medicare Advantage, or private insurance benefits before a hospitalization can save valuable time and money.
- Questions every family should ask the hospital discharge planner before a loved one is released.
- Common reasons seniors are readmitted to the hospital—and practical steps to help reduce that risk.
- How proper communication between physicians, care teams, caregivers, and family members leads to better outcomes.
- Warning signs to watch for after discharge that may require immediate medical attention.
- How to prepare the home for a safe recovery and support long-term independence.
- Resources available to help seniors and caregivers successfully navigate the transition from hospital to home or another level of care.
Whether you're caring for an aging parent, planning ahead for your own future, or helping a loved one recover after a hospital stay, this episode provides valuable guidance that every family should know before a healthcare emergency occurs.
