1
00:00:00.030 --> 00:00:02.089
Thank you for tuning in to Help for HD Live.
2
00:00:02.250 --> 00:00:08.740
This podcast is made possible by Teva Pharmaceuticals, Neurocrime Biosciences, Sarepta, Unicure,
3
00:00:08.800 --> 00:00:11.500
and the Griffin Foundation. I am your host, Katie Jackson.
4
00:00:11.880 --> 00:00:14.539
And today we have Wes back with us. So in
5
00:00:14.599 --> 00:00:17.280
our last show, we talked about timing of testing and
6
00:00:17.300 --> 00:00:20.399
the testing process. But today we are going to get
7
00:00:20.420 --> 00:00:23.679
a little bit more into the weeds of it slightly.
8
00:00:24.739 --> 00:00:27.570
So thank you for coming back with us today, Wes.
9
00:00:28.489 --> 00:00:29.789
Absolutely. Happy to be here.
10
00:00:30.359 --> 00:00:35.000
Let's talk about personal and family impact. So first question,
11
00:00:35.380 --> 00:00:39.039
how can knowing your genetic status for Huntington's disease impact
12
00:00:39.299 --> 00:00:40.140
you personally?
13
00:00:40.159 --> 00:00:44.729
Yeah, gosh, that's a really great question. And often kind
14
00:00:44.750 --> 00:00:47.570
of a big driver in making this decision, right, is
15
00:00:47.609 --> 00:00:53.630
really thinking about how does this information affect you personally
16
00:00:53.750 --> 00:00:58.439
and your world? So individually, you know, personally, how does
17
00:00:58.460 --> 00:01:00.560
this affect you? And And I think that there are
18
00:01:01.179 --> 00:01:06.140
some things to consider as far as emotional effects. There's
19
00:01:06.260 --> 00:01:10.879
interpersonal effects, whether that's with your partner or your spouse,
20
00:01:11.040 --> 00:01:13.680
or if that's having to do with your family, but
21
00:01:13.760 --> 00:01:18.450
really kind of centering on the individual can be really
22
00:01:18.510 --> 00:01:22.469
heavy information to receive, right? And that's whether that test
23
00:01:22.510 --> 00:01:27.769
comes back positive or negative, you know, I think there's
24
00:01:27.810 --> 00:01:29.769
a lot of focus on if that test were to
25
00:01:29.810 --> 00:01:32.430
come back positive, how heavy that information can be and
26
00:01:32.489 --> 00:01:36.799
what kind of effects can be there. And kind of
27
00:01:37.340 --> 00:01:39.680
thinking about the future, what that would look like for
28
00:01:39.719 --> 00:01:44.239
you and kind of drawing conclusions from lived experiences with
29
00:01:44.260 --> 00:01:48.650
family members that, you know, maybe have you've watched deal
30
00:01:48.670 --> 00:01:52.290
with Huntington's disease. That can be especially difficult drawing that
31
00:01:52.329 --> 00:01:54.530
parallel to your own life and kind of wondering what
32
00:01:54.549 --> 00:01:57.579
that will look like for you in the future. Additionally,
33
00:01:57.680 --> 00:02:01.329
receiving a negative result, sometimes people don't expect that to
34
00:02:01.359 --> 00:02:03.329
be as heavy as it can be. And it really
35
00:02:03.349 --> 00:02:07.310
can be when, you know, especially heightened by having maybe
36
00:02:07.349 --> 00:02:11.129
another at risk sibling. And you're kind of thinking about
37
00:02:11.590 --> 00:02:15.099
receiving these results. And you're feeling this sense of relief
38
00:02:15.199 --> 00:02:18.240
and celebration. And you're like, man, I don't have to
39
00:02:18.280 --> 00:02:21.479
worry about this. But then you're reminded of your sibling
40
00:02:21.520 --> 00:02:24.860
that's still at risk. And that's a challenging position to
41
00:02:24.900 --> 00:02:27.590
be in. And Do I share this information? Do I
42
00:02:27.650 --> 00:02:31.990
feel sort of guilty about having this result when they're
43
00:02:32.030 --> 00:02:34.550
still at risk? Or whenever I have another family member
44
00:02:34.590 --> 00:02:38.379
that's living with Huntington's disease and experiencing it right now,
45
00:02:38.860 --> 00:02:42.240
there's just a whole mix of things that can go
46
00:02:42.300 --> 00:02:46.780
along with receiving your results that really, I think, it
47
00:02:46.819 --> 00:02:49.680
really highlights the importance of talking it through with someone,
48
00:02:49.719 --> 00:02:54.250
a genetic counselor or someone who's really specialized with Huntington's
49
00:02:54.310 --> 00:02:58.629
disease or someone really knowledgeable in the community. Yeah.
50
00:02:59.389 --> 00:03:03.370
In what ways might test results affect family relationships and dynamics?
51
00:03:04.330 --> 00:03:07.490
Great question. Yeah. So this can come up in different ways.
52
00:03:07.610 --> 00:03:10.729
And one of which is, you know, mentioning, thinking about
53
00:03:11.310 --> 00:03:14.400
the at-risk siblings and thinking about how that can, you know,
54
00:03:14.439 --> 00:03:17.340
come into play or thinking about sibling who has already
55
00:03:17.400 --> 00:03:21.000
tested and you're testing and getting your results and maybe
56
00:03:21.020 --> 00:03:24.900
your results are discordant with theirs, or maybe you know,
57
00:03:24.939 --> 00:03:27.800
you both tested positive and you're sharing this information with them.
58
00:03:28.199 --> 00:03:31.439
There's all kinds of things to consider there. There's also,
59
00:03:31.750 --> 00:03:34.770
you know, when we think about another dynamic, it's sharing
60
00:03:34.810 --> 00:03:38.009
that information with your parent. And, you know, that can
61
00:03:38.050 --> 00:03:42.009
be all sorts of different scenarios too, where are you
62
00:03:42.050 --> 00:03:47.090
sharing this information with the parent that is experiencing Huntington's disease?
63
00:03:47.240 --> 00:03:50.740
And does that maybe bring up some feelings of guilt
64
00:03:50.840 --> 00:03:54.879
for that parent or, you know, some resentment or, you know,
65
00:03:54.979 --> 00:03:58.659
all kinds of different things that can come to surface. Similarly,
66
00:03:58.719 --> 00:04:03.229
sharing that information with maybe your parent that's not experiencing
67
00:04:03.270 --> 00:04:06.729
Huntington's disease can be tricky too, because of what they've
68
00:04:06.830 --> 00:04:11.569
lived with the parent that has lived with Huntington's disease.
69
00:04:11.909 --> 00:04:15.759
You know, I think one of the important things about
70
00:04:15.819 --> 00:04:19.079
genetic counseling and why, you know, I recommend folks try
71
00:04:19.120 --> 00:04:21.839
to talk with a genetic counselor about this decision is
72
00:04:22.430 --> 00:04:25.509
is exactly this reason. So there's already a lot to
73
00:04:25.550 --> 00:04:29.040
consider as far as individual effects and how it may
74
00:04:29.079 --> 00:04:33.459
affect you personally. But beyond that, it's interpersonal. It's thinking
75
00:04:33.519 --> 00:04:36.360
about how does this affect people other than me, people
76
00:04:37.240 --> 00:04:39.500
close to me and people I love. Who do I
77
00:04:39.560 --> 00:04:42.040
wanna know and who do I not wanna know? And
78
00:04:42.079 --> 00:04:44.639
these kinds of questions I think are really important to ask.
79
00:04:46.399 --> 00:04:50.100
How do you counsel individuals about the possibility of passing
80
00:04:50.139 --> 00:04:52.060
Huntington's disease onto their children?
81
00:04:53.730 --> 00:04:58.889
That's a great question. The short answer is sharing with
82
00:04:58.949 --> 00:05:04.069
folks that if you've inherited this genetic cause of Huntington's disease,
83
00:05:04.149 --> 00:05:07.779
if you have this genetic cause of Huntington's disease, any
84
00:05:07.819 --> 00:05:11.420
biological children that you have will be at a 50-50
85
00:05:11.500 --> 00:05:16.720
risk of inheriting that genetic cause of ADHD. Unless you
86
00:05:16.759 --> 00:05:21.939
were to take some biological measures, whether that's IVF and
87
00:05:22.160 --> 00:05:26.860
genetic testing of embryos prior to implantation and other scientific
88
00:05:27.379 --> 00:05:30.839
marvels like that, there's a 50-50 risk of each child
89
00:05:31.100 --> 00:05:37.620
of an affected parent. Beyond that, with the answer being
90
00:05:37.639 --> 00:05:40.660
a little more complicated, I think is thinking about the
91
00:05:40.740 --> 00:05:45.680
psychosocial effects, thinking about the emotional side of talking with
92
00:05:45.720 --> 00:05:49.199
someone about the possibility of them having this genetic cause
93
00:05:49.240 --> 00:05:51.540
of HD and what that could mean for their children
94
00:05:51.579 --> 00:05:55.060
and the potential risk there is often a really hard
95
00:05:55.170 --> 00:05:58.870
conversation to have when we're talking about testing. So when
96
00:05:58.889 --> 00:06:01.310
we're talking about these family dynamics, like I was earlier
97
00:06:01.370 --> 00:06:05.100
with siblings or parents, When you think about it from
98
00:06:05.139 --> 00:06:08.779
the perspective of the individual already having children and they're
99
00:06:08.800 --> 00:06:11.040
thinking about this potential risk for them, that can be
100
00:06:11.060 --> 00:06:12.240
a really tough conversation.
101
00:06:12.259 --> 00:06:15.019
It came out of so left field for us. And
102
00:06:15.050 --> 00:06:17.949
I remember like learning that it was like more scary.
103
00:06:18.009 --> 00:06:19.709
I think for my husband that he was thinking he
104
00:06:19.750 --> 00:06:22.970
was testing to see if his kids were at risk. Right.
105
00:06:23.029 --> 00:06:25.970
And like, cause that's when we started doctor Googling and learning,
106
00:06:26.009 --> 00:06:27.709
we thought, oh my gosh, they're at risk too. When
107
00:06:27.750 --> 00:06:30.550
we started learning, if someone finds out their genetic status,
108
00:06:30.980 --> 00:06:34.800
How might their information directly reveal the status of untested
109
00:06:34.839 --> 00:06:38.180
family members, such as a parent who hasn't been tested?
110
00:06:38.199 --> 00:06:42.579
Yeah, this does come up. So sometimes, like, let's talk
111
00:06:42.639 --> 00:06:46.160
about this is something that comes up called 25% risk,
112
00:06:46.699 --> 00:06:50.670
where let's say there's an individual who is curious about
113
00:06:50.699 --> 00:06:56.040
genetic testing for Huntington's disease. They know that their grandparent
114
00:06:56.620 --> 00:07:00.339
is affected by HD or has passed from HD, but
115
00:07:00.459 --> 00:07:05.370
their parent, so the kind of middle generation there, their
116
00:07:05.490 --> 00:07:08.949
mother or their father has not tested, but they are
117
00:07:08.990 --> 00:07:13.269
at risk of Huntington's. They're at the 50-50 risk. But
118
00:07:13.350 --> 00:07:17.490
that parent's child is the one who's curious about testing.
119
00:07:17.550 --> 00:07:20.769
Maybe they're in a stage of family planning. Maybe they're
120
00:07:20.790 --> 00:07:24.110
wanting to think about having their own kids or maybe
121
00:07:24.149 --> 00:07:26.649
they just want to know for themselves. They're just sort
122
00:07:26.670 --> 00:07:28.529
of in a position where they're like, I want to
123
00:07:28.589 --> 00:07:31.470
know if I'm at risk for Huntington's. And maybe they
124
00:07:31.490 --> 00:07:34.009
talk to their parent and they say, hey, if you
125
00:07:34.040 --> 00:07:36.269
test and you test negative, I don't have to worry
126
00:07:36.310 --> 00:07:40.920
about it. But that parent just does not want to know,
127
00:07:41.060 --> 00:07:47.240
which is fine, right? That person seeking testing, if they
128
00:07:47.279 --> 00:07:50.180
were to test positive, that would of course mean that
129
00:07:50.199 --> 00:07:54.139
their parent is also positive. And that can be a
130
00:07:55.019 --> 00:07:59.360
really difficult situation, especially if that parent really doesn't want
131
00:07:59.379 --> 00:08:02.459
to know their own status. So this is definitely something
132
00:08:02.500 --> 00:08:07.490
to explore before you undergo testing is thinking about, you know,
133
00:08:07.519 --> 00:08:11.029
thinking ahead and thinking about, is this information that I
134
00:08:11.069 --> 00:08:13.610
share with them or do I have a way that
135
00:08:13.750 --> 00:08:17.389
I keep this to myself or me and my spouse
136
00:08:17.449 --> 00:08:20.569
or kind of keep it away from them and as
137
00:08:20.629 --> 00:08:24.569
healthy of a way as possible? what does that realistically
138
00:08:24.629 --> 00:08:28.110
look like? Cause sometimes people approach it and they say, well,
139
00:08:28.149 --> 00:08:30.040
I just want to know. And they haven't really thought
140
00:08:30.079 --> 00:08:32.539
about that. And they've sort of thought about it like, well,
141
00:08:32.720 --> 00:08:34.120
I want to know, and I'm going to find out.
142
00:08:34.139 --> 00:08:36.019
And when I find out, I guess they're going to
143
00:08:36.059 --> 00:08:40.379
find out, take, take a breath on that one, you know,
144
00:08:40.399 --> 00:08:43.590
like maybe think about it a little more and just
145
00:08:43.610 --> 00:08:47.570
kind of approach it with an intention and, Thoughtfulness, you know,
146
00:08:47.629 --> 00:08:51.210
that's not to say that's not your information to learn, right?
147
00:08:51.289 --> 00:08:53.210
Of course, you have your own life, you need to
148
00:08:53.230 --> 00:08:56.220
do your own things. But maybe just thinking about that
149
00:08:56.259 --> 00:08:59.639
parent that's at risk and how to approach that. Sometimes,
150
00:09:00.419 --> 00:09:03.240
this might be helpful to share. Sometimes what people will
151
00:09:03.259 --> 00:09:08.710
do is they will approach this conversation with their parent as, hey,
152
00:09:08.850 --> 00:09:12.070
I am really thinking about testing. Like, I'm really thinking,
153
00:09:12.110 --> 00:09:14.690
I think I'm going to do it. Is that information
154
00:09:14.789 --> 00:09:17.470
that you want to know or not? Like, should I
155
00:09:17.750 --> 00:09:19.350
not share it with you or should I? And just
156
00:09:19.389 --> 00:09:24.720
have that direct conversation. You could have already tested at
157
00:09:24.759 --> 00:09:28.080
that point, and maybe you know your answer, but you're
158
00:09:28.200 --> 00:09:32.029
gauging them to see if that's a comfortable conversation, that
159
00:09:32.070 --> 00:09:35.950
they are open to receiving your information. And if they are,
160
00:09:36.049 --> 00:09:38.629
then maybe that's a different conversation. You guys talk about it.
161
00:09:38.990 --> 00:09:41.309
if they're very much against it and they put up
162
00:09:41.370 --> 00:09:43.960
their guardrails and they're like, I don't want to talk
163
00:09:44.000 --> 00:09:46.899
about this, then you know that that's something that maybe
164
00:09:46.919 --> 00:09:49.460
you guys don't talk about. And maybe you're like, well,
165
00:09:49.480 --> 00:09:51.840
I'm glad I didn't bring up that. I've tested, you know,
166
00:09:51.919 --> 00:09:53.350
so just food for thought.
167
00:09:54.309 --> 00:09:56.470
Yeah, for sure. What about identical twins?
168
00:09:57.850 --> 00:10:01.840
Identical twins is really tough. Yeah. That, that's come up too.
169
00:10:02.299 --> 00:10:06.809
If they are truly identical twins, then that means you're
170
00:10:06.830 --> 00:10:10.990
testing and And if you get your results, you're finding
171
00:10:11.090 --> 00:10:13.879
out for your twin as well. And this is very
172
00:10:13.960 --> 00:10:17.960
similar to that situation with the parent that maybe wants
173
00:10:18.000 --> 00:10:20.879
to know, maybe doesn't want to know. It's very important
174
00:10:20.899 --> 00:10:24.419
to have a clear conversation with your twin and say,
175
00:10:24.990 --> 00:10:28.850
here's the situation. I want to know this information. If
176
00:10:30.389 --> 00:10:34.899
I find out, it's basically like testing you too. Is
177
00:10:34.950 --> 00:10:38.090
this information that you want to know or not? Or
178
00:10:38.169 --> 00:10:41.169
how do we navigate this? I think it's, you know,
179
00:10:41.190 --> 00:10:44.769
a matter of just being intentional and having these open conversations.
180
00:10:45.610 --> 00:10:48.690
Yeah, absolutely. It's so, to me, it's so crazy because
181
00:10:49.210 --> 00:10:52.649
like for an identical twin, you know, we hear in
182
00:10:52.710 --> 00:10:55.730
clinics that they won't test siblings together. So they'll make
183
00:10:55.769 --> 00:10:58.110
them test like X amount of time apart from each other.
184
00:10:58.149 --> 00:11:00.070
We won't give you your, you can test the same time,
185
00:11:00.129 --> 00:11:02.509
but we won't give you the results within a room together.
186
00:11:02.549 --> 00:11:03.330
We hear that often.
187
00:11:03.350 --> 00:11:03.529
Yeah.
188
00:11:04.080 --> 00:11:07.000
But like an identical twin, you almost feel like that
189
00:11:07.039 --> 00:11:09.139
has to be an exception to the rule. I mean,
190
00:11:10.190 --> 00:11:12.269
why would you not tell them together if they both
191
00:11:12.309 --> 00:11:15.350
want the knowledge? Because they're going to tell each other
192
00:11:15.389 --> 00:11:17.570
and find out anyways. It's like, it's such a different,
193
00:11:17.610 --> 00:11:21.690
I mean, I know it's rare, but it's there, right? Yeah. Yeah.
194
00:11:21.710 --> 00:11:25.080
That's a tricky one. I would say, you know, sometimes
195
00:11:25.539 --> 00:11:28.399
the way I always try to approach it the way
196
00:11:28.440 --> 00:11:31.220
that I described, which is, you know, essentially you're going
197
00:11:31.240 --> 00:11:34.049
to get that result. It's going to inform everything. the
198
00:11:34.090 --> 00:11:37.929
other person's result, but when the other twin does want
199
00:11:37.960 --> 00:11:42.059
to test for themselves, I always leave open the possibility
200
00:11:42.100 --> 00:11:44.740
that maybe they're not identical twins.
201
00:11:44.940 --> 00:11:48.059
Let's get into a little bit about supports and counseling.
202
00:11:48.460 --> 00:11:53.059
What emotional and psychological support resources are available and what
203
00:11:53.120 --> 00:11:56.120
should individuals consider when undergoing testing?
204
00:11:57.220 --> 00:12:00.250
Yeah, I think that that's something important to focus on
205
00:12:00.389 --> 00:12:04.509
if you're thinking about diving into the consideration of testing
206
00:12:04.629 --> 00:12:08.169
is think about your support network. And I think that
207
00:12:08.269 --> 00:12:12.200
that term sometimes can get thrown around, but it's true.
208
00:12:12.720 --> 00:12:14.559
Think about who do you want to go on this
209
00:12:14.600 --> 00:12:17.539
journey with you? Is that a spouse or partner or
210
00:12:20.870 --> 00:12:24.389
is it a close friend that you've confided in through
211
00:12:24.409 --> 00:12:28.629
this process? Is it someone just, you know, someone that
212
00:12:28.649 --> 00:12:31.169
comes to mind when you think about, I want to test,
213
00:12:31.389 --> 00:12:36.120
you know, what does this look like? I think it's beyond.
214
00:12:36.580 --> 00:12:39.299
So that's, that's sort of your personal world when you're
215
00:12:39.320 --> 00:12:42.120
thinking about your support network and, and how to go
216
00:12:42.139 --> 00:12:47.049
through this journey of testing beyond that. Think about like, okay,
217
00:12:47.110 --> 00:12:52.509
but what else? Like what, whenever I'm, having tested and
218
00:12:52.549 --> 00:12:56.360
I'm sort of ready to turn to other folks, maybe
219
00:12:56.409 --> 00:13:03.279
professional help. I think it's important. I'm always a big surprise,
220
00:13:03.340 --> 00:13:07.179
but I'm a big champion of therapy and talking to
221
00:13:07.220 --> 00:13:11.720
a therapist. And sometimes I think that gets tied up
222
00:13:11.779 --> 00:13:13.960
with folks feeling like they have to be depressed or
223
00:13:14.019 --> 00:13:17.039
anxious or something like that to have a therapist. And
224
00:13:17.059 --> 00:13:21.460
that's not necessarily true. So just in life in general,
225
00:13:21.480 --> 00:13:23.440
it can be helpful to have a therapist. So it's
226
00:13:23.500 --> 00:13:26.659
especially helpful if you're trying to make a big decision
227
00:13:26.700 --> 00:13:30.889
like this, such as undergoing testing for HD and what
228
00:13:30.950 --> 00:13:35.309
life looks like after that. So genetic counselors are there
229
00:13:35.350 --> 00:13:37.750
to help you through this process and help talk you
230
00:13:37.789 --> 00:13:41.210
through what it looks like and things to consider. But
231
00:13:42.110 --> 00:13:47.340
a therapist, like an actual psychotherapist or counselor, can be
232
00:13:47.360 --> 00:13:50.960
a really good resource to lean on about all of
233
00:13:51.039 --> 00:13:55.279
your inner world, your interpersonal world with your family dynamics
234
00:13:55.340 --> 00:13:58.490
and how you think this might affect your life and
235
00:13:58.519 --> 00:14:01.929
your work and all these sorts of different things. So
236
00:14:02.070 --> 00:14:04.429
it's a really good idea maybe to get connected with
237
00:14:04.470 --> 00:14:09.029
these things. Sometimes people get hung up on feeling like,
238
00:14:10.129 --> 00:14:12.480
I want a therapist, but I want a therapist that
239
00:14:12.500 --> 00:14:15.639
already knows about Huntington's disease. Because I don't want to
240
00:14:15.679 --> 00:14:18.080
have to explain it to them and, you know, they
241
00:14:18.120 --> 00:14:20.860
should just know about it. And I hear where you're
242
00:14:20.899 --> 00:14:24.570
coming from, if that's you. I do also wish that
243
00:14:24.590 --> 00:14:28.590
that was the case. But more important than anything is
244
00:14:28.710 --> 00:14:31.190
try to find a therapist that you truly connect with
245
00:14:31.309 --> 00:14:34.059
and you feel like is someone that you can confide
246
00:14:34.200 --> 00:14:36.159
in and you feel like has your back and is
247
00:14:36.259 --> 00:14:40.399
in your corner. Because, you know, more likely than not,
248
00:14:40.679 --> 00:14:42.620
that person is going to be the one that you
249
00:14:42.679 --> 00:14:45.100
want in your corner and that you want having your
250
00:14:45.120 --> 00:14:48.659
back and is willing to learn about Huntington's disease and
251
00:14:48.700 --> 00:14:50.860
really kind of trying to get in there. And if
252
00:14:50.879 --> 00:14:53.070
you're listening to this and you're thinking like, well, I
253
00:14:53.110 --> 00:14:55.950
do have a therapist, they don't know about Huntington's disease,
254
00:14:56.710 --> 00:14:58.809
send them my way. I'm happy to talk to your
255
00:14:58.850 --> 00:15:03.590
therapist and educate them about Huntington's disease. Send them to Katie, Katrina,
256
00:15:03.629 --> 00:15:04.330
send them to Help4HD.
257
00:15:04.049 --> 00:15:04.330
Com.
258
00:15:05.669 --> 00:15:11.129
They are going to find this information to help you. So, yeah,
259
00:15:11.250 --> 00:15:15.120
I think that that's really important. And social workers, too.
260
00:15:15.240 --> 00:15:18.899
Always keep a lookout for social workers. You can connect
261
00:15:18.919 --> 00:15:21.549
with Help for HD, of course, with Jamie. You can
262
00:15:21.600 --> 00:15:24.419
go to your HDSA center of excellence. They generally have
263
00:15:24.460 --> 00:15:28.039
a social worker tied to their clinic that is, you know,
264
00:15:28.100 --> 00:15:30.960
knowledgeable about that local or regional space.
265
00:15:31.580 --> 00:15:34.720
So my kids, I listen, even though I go to
266
00:15:34.740 --> 00:15:37.340
all these guys, we host these conferences. I actually sit
267
00:15:37.379 --> 00:15:40.320
and listen to almost every session and I take notes
268
00:15:40.360 --> 00:15:43.299
because I'm an HD family. So, you know, and one
269
00:15:43.360 --> 00:15:46.840
thing that we talked about early on was, was counseling
270
00:15:46.940 --> 00:15:51.149
for my children. So I had my children established at like,
271
00:15:51.669 --> 00:15:54.929
for instance, my, my oldest Madison, she was established with
272
00:15:55.190 --> 00:15:59.789
a counselor at 15 and she's kept that counselor her
273
00:15:59.830 --> 00:16:02.330
whole entire, now she's 23 and she still has that
274
00:16:02.370 --> 00:16:05.850
same counselor. So even though she doesn't know, yes, about
275
00:16:05.909 --> 00:16:09.309
Huntington's disease directly, she went through her dad's, you know, journey,
276
00:16:09.750 --> 00:16:13.549
her dad passing all of these milestones, Madison, you know,
277
00:16:13.570 --> 00:16:18.059
starting college, Madison finishing high school. me getting remarried, right?
278
00:16:18.799 --> 00:16:22.929
All these different things that went on in Madison's life, um,
279
00:16:23.370 --> 00:16:26.669
she's followed. And then, so Madison, as she goes, you know,
280
00:16:26.710 --> 00:16:30.289
as she's going through the testing process, she has that support, um,
281
00:16:30.429 --> 00:16:32.549
to be able to go and see this very, you know,
282
00:16:32.830 --> 00:16:35.950
so I'm so glad that I started that really young
283
00:16:36.110 --> 00:16:40.230
and I know it's expensive. I know like it, But,
284
00:16:40.730 --> 00:16:42.750
you know, it's and it's hard to get a counselor.
285
00:16:42.789 --> 00:16:44.960
Let's all be honest. You know, you call mental health
286
00:16:44.980 --> 00:16:48.059
services on your insurance and you sit for six months
287
00:16:48.120 --> 00:16:50.399
waiting for someone to have an opening on their schedule.
288
00:16:50.419 --> 00:16:54.620
But yeah, it's so important. I really do believe it's important.
289
00:16:55.039 --> 00:16:57.340
And sometimes an outsider that knows nothing about you, she
290
00:16:57.399 --> 00:16:58.639
isn't a bad thing either.
291
00:16:59.720 --> 00:17:02.659
Absolutely. That is a really great point, Katie. Sometimes just
292
00:17:02.679 --> 00:17:06.180
getting that outside perspective of someone that's not all wrapped
293
00:17:06.240 --> 00:17:11.200
up in it can be really, really helpful. So, yeah, yeah. Yeah.
294
00:17:11.359 --> 00:17:13.880
I think it's anyone that understands like grief and those things.
295
00:17:13.940 --> 00:17:18.460
And most counselors do, right? Yeah. So we'll wrap up
296
00:17:18.500 --> 00:17:24.609
with reproductive choices. So, yeah. What options exist for family planning,
297
00:17:25.789 --> 00:17:32.289
such as pre-implantation? You say it for me.
298
00:17:32.869 --> 00:17:36.890
It's a crazy one. Pre-implantation genetic testing. Yeah.
299
00:17:38.049 --> 00:17:41.420
Yeah. prenatal testing for people who are at risk for
300
00:17:41.440 --> 00:17:42.279
Huntington's disease.
301
00:17:43.140 --> 00:17:43.759
Absolutely.
302
00:17:43.839 --> 00:17:44.099
Yeah.
303
00:17:44.180 --> 00:17:46.690
It's funny that, you know, getting into this whole world,
304
00:17:48.150 --> 00:17:50.049
the world that we're in, I still have trouble saying
305
00:17:50.170 --> 00:17:55.410
neurodegenerative and it's, oh my gosh, it comes up. Oh
306
00:17:55.430 --> 00:18:01.420
my gosh. Anyway, options, family planning options. There are a
307
00:18:01.579 --> 00:18:04.380
number of options. The first option that I always want
308
00:18:04.400 --> 00:18:07.559
to make sure that people consider, and sometimes they feel
309
00:18:07.579 --> 00:18:09.519
a certain way about, but you know, that's up to
310
00:18:09.559 --> 00:18:12.619
you to each their own. Again, it's just the good
311
00:18:12.660 --> 00:18:15.920
old fashioned way. You can just have kids if you
312
00:18:15.960 --> 00:18:18.119
want to, you know, I think that there's a lot
313
00:18:18.160 --> 00:18:23.170
of judgment sometimes about, you know, knowing your HD risk
314
00:18:23.210 --> 00:18:26.210
and having kids, but this is truly your decision, you know,
315
00:18:26.369 --> 00:18:29.569
and I think it's something to think about, right? When
316
00:18:29.609 --> 00:18:33.549
we're weighing out all the options, let's consider all the options.
317
00:18:33.630 --> 00:18:35.450
And if that is your choice, and that's the way
318
00:18:35.470 --> 00:18:39.630
you're going, That's okay. That's your choice. But when we're
319
00:18:39.670 --> 00:18:43.900
talking about ways of mitigating risk or, you know, lowering
320
00:18:43.960 --> 00:18:47.759
or eliminating risk of Huntington's disease risk for family planning
321
00:18:47.799 --> 00:18:50.400
and pregnancies and these kinds of things, there are a
322
00:18:50.420 --> 00:18:54.259
number of options. So sometimes what folks will do is
323
00:18:54.759 --> 00:18:58.319
look into adoption. First of all, maybe adopting kids is
324
00:18:58.380 --> 00:19:00.619
the route to go. I have Personally, a lot of
325
00:19:00.650 --> 00:19:03.759
friends that were adopted, they're really lovely friends and come
326
00:19:03.799 --> 00:19:06.039
to mind whenever I think about adoption. So I think
327
00:19:06.099 --> 00:19:11.380
it's really excellent to be a parent adopting. There are
328
00:19:11.480 --> 00:19:15.059
also situations, so depending on which parent is at risk,
329
00:19:15.619 --> 00:19:20.240
you can have donor egg or donor sperm. If, you know,
330
00:19:20.279 --> 00:19:23.349
having biological children is important to you and you want to,
331
00:19:23.480 --> 00:19:25.589
you know, have half of the DNA be from one
332
00:19:25.650 --> 00:19:29.630
parent and that kind of thing, that's a possibility. There
333
00:19:29.690 --> 00:19:33.240
is also, as Katie mentioned earlier, this possibility for what
334
00:19:33.279 --> 00:19:36.279
we call, well, the name always changes, but generally it
335
00:19:36.319 --> 00:19:42.609
kind of revolves around IVF-PGT, which is essentially in vitro
336
00:19:42.660 --> 00:19:49.670
fertilization and pre-implantation genetic testing, meaning genetic testing of embryos.
337
00:19:49.970 --> 00:19:53.390
so the sperm and the egg combined, genetic testing of embryos,
338
00:19:54.059 --> 00:19:57.700
and then implanting those embryos that are free of risk
339
00:19:57.740 --> 00:20:04.259
of Huntington's disease. And this process, this can actually be
340
00:20:04.359 --> 00:20:11.380
done through something called non-disclosure testing, meaning if you personally
341
00:20:11.450 --> 00:20:14.539
don't want to know your own genetic status, you don't
342
00:20:14.559 --> 00:20:16.680
want to know if you've inherited that genetic cause of
343
00:20:16.720 --> 00:20:19.920
Huntington's disease, but you want to move forward with family
344
00:20:19.960 --> 00:20:23.230
planning and having pregnancies free of risk of HD, you
345
00:20:23.289 --> 00:20:28.150
can do that. You can have IVF, PGT through non-disclosure
346
00:20:28.170 --> 00:20:33.759
testing where they essentially, the fertility clinic identifies those embryos,
347
00:20:34.299 --> 00:20:36.799
only implants those embryos free of risk of HD, and
348
00:20:36.839 --> 00:20:40.039
they don't tell you if they found any embryos that
349
00:20:40.279 --> 00:20:44.240
are actually at risk of HD. That way, you don't
350
00:20:44.299 --> 00:20:48.089
have to learn your status before you're ready. And that's
351
00:20:48.160 --> 00:20:51.349
something that a lot of people don't know is possible.
352
00:20:51.750 --> 00:20:54.950
And further, if that is something that you're interested in,
353
00:20:54.970 --> 00:20:58.859
I would just caution you to reach out to that
354
00:20:58.920 --> 00:21:03.869
fertility clinic directly and just be very, very straightforward, direct
355
00:21:03.930 --> 00:21:06.490
and open about it and say, here's the situation. I
356
00:21:06.490 --> 00:21:11.529
do not want to know my own genetic status for
357
00:21:11.569 --> 00:21:14.319
Huntington's disease, but we want to make sure we have
358
00:21:14.339 --> 00:21:17.180
pregnancies that are free of risk of HD. That way
359
00:21:17.220 --> 00:21:20.019
they don't accidentally somehow put it in your record. I
360
00:21:20.039 --> 00:21:23.650
would just not be afraid of just overstepping and being
361
00:21:23.710 --> 00:21:26.000
as clear as day about it, if that's what you're
362
00:21:26.019 --> 00:21:29.460
concerned about, just to be really, really safe and sure.
363
00:21:30.579 --> 00:21:32.960
So there's a number of options, but yeah.
364
00:21:32.980 --> 00:21:36.599
Right. I, we, I just had a friend, well, this
365
00:21:36.619 --> 00:21:37.799
is a couple of years ago. I had a friend
366
00:21:37.940 --> 00:21:40.140
and she, I'm so glad this is why I just
367
00:21:40.180 --> 00:21:42.069
love Wes. He just said, Hey, look, there's just like
368
00:21:42.109 --> 00:21:44.589
the old fashioned way that don't, you know, there's judgment
369
00:21:44.609 --> 00:21:46.950
in this. And I think the people that judge the
370
00:21:46.990 --> 00:21:49.009
most are the people within the HC community. They're the
371
00:21:49.089 --> 00:21:52.480
worst at this. And I, I, I had a friend
372
00:21:52.660 --> 00:21:56.970
and her, her child had passed from JT and her, her, um,
373
00:21:57.329 --> 00:22:00.009
she was pregnant, um, by her and her and her
374
00:22:00.049 --> 00:22:03.009
husband were having another baby who was symptomatic. And she
375
00:22:03.049 --> 00:22:04.829
came to one of our events and she was like,
376
00:22:04.890 --> 00:22:06.569
I was kind of embarrassed to come here. Cause I
377
00:22:06.690 --> 00:22:08.210
know people are looking at me and I was like,
378
00:22:08.609 --> 00:22:09.849
you're having a baby.
379
00:22:09.970 --> 00:22:11.890
That's a beautiful thing.
380
00:22:12.029 --> 00:22:17.109
Like it's, you know, I remember Mike was fully symptomatic
381
00:22:17.150 --> 00:22:18.630
when I delivered Macy.
382
00:22:18.730 --> 00:22:22.279
I put him, put her in his arms and, Yeah.
383
00:22:23.079 --> 00:22:25.140
He gave her, I swear he lived a couple more
384
00:22:25.160 --> 00:22:27.369
years because of her and loved her. And he was
385
00:22:27.400 --> 00:22:30.069
a wonderful father, even symptomatic with Korea and everything. He
386
00:22:30.089 --> 00:22:33.630
was an amazing father. Macy is an amazing human. So
387
00:22:33.650 --> 00:22:37.349
I don't, the judgment is just, it's, but yes, I
388
00:22:37.390 --> 00:22:38.970
do believe this is, I love what you said. This
389
00:22:39.029 --> 00:22:43.690
is your journey. So yeah, completely. So let's finish with
390
00:22:43.829 --> 00:22:47.009
how do you help patients discuss genetic risks and reproductive
391
00:22:47.029 --> 00:22:48.809
choices with their partners?
392
00:22:49.880 --> 00:22:53.079
Yeah, that's a good question that comes up. So I
393
00:22:53.119 --> 00:22:56.440
would say, you know, it's, it's really important to have
394
00:22:56.480 --> 00:23:01.029
these conversations. And it's really important to, if you're having
395
00:23:01.069 --> 00:23:04.519
children together to really find common ground and really decide
396
00:23:04.980 --> 00:23:06.940
what it is that you want to do. You know,
397
00:23:07.059 --> 00:23:10.109
one thing is, Well, one thing that comes to mind
398
00:23:10.190 --> 00:23:14.400
is when we talk about IVF and pre-implantation genetic testing
399
00:23:14.460 --> 00:23:17.660
and this option that's available, I think sometimes it can
400
00:23:17.680 --> 00:23:21.480
get tossed around like it's just this easy option when
401
00:23:21.619 --> 00:23:25.640
it is extremely difficult on the prospective mother to undergo
402
00:23:25.700 --> 00:23:29.609
these rounds of IVF and egg retrieval and these things.
403
00:23:29.970 --> 00:23:34.970
And it's also not a guarantee. It's really... the chances
404
00:23:35.049 --> 00:23:41.519
of having successful egg retrievals and successful implantation and pregnancies
405
00:23:41.579 --> 00:23:47.240
from IVF are already challenged. So I think that you're
406
00:23:47.299 --> 00:23:51.140
having a big conversation already, but it does warrant an
407
00:23:51.420 --> 00:23:56.630
even bigger conversation of managing expectations and trying to think
408
00:23:56.710 --> 00:23:59.509
about what exactly it's gonna look like if that's the
409
00:23:59.630 --> 00:24:04.789
choice that you make. And then further into the weeds
410
00:24:04.890 --> 00:24:07.359
and kind of getting into the really granular details and
411
00:24:07.410 --> 00:24:10.880
nitty gritty, since we are talking about Huntington's disease is
412
00:24:11.039 --> 00:24:15.000
talking about if you're doing IVF and genetic testing of
413
00:24:15.039 --> 00:24:19.339
those embryos, what is your number threshold? So when you're
414
00:24:19.640 --> 00:24:22.839
talking about, you know, doing these genetic tests of HD,
415
00:24:24.039 --> 00:24:28.460
what exactly warrants an embryo that is being implanted versus
416
00:24:28.500 --> 00:24:31.119
an embryo that is not, you know, if you had
417
00:24:31.579 --> 00:24:34.259
an embryo that was in the intermediate range, as we've
418
00:24:34.279 --> 00:24:38.200
talked about before, where that CAG is somewhere between 27
419
00:24:38.200 --> 00:24:43.230
to 35, is that some, is that, you know, someone who,
420
00:24:43.529 --> 00:24:46.849
an embryo that gets implanted and, you know, are you
421
00:24:46.950 --> 00:24:50.250
able to keep that number away from yourself? You know,
422
00:24:50.309 --> 00:24:53.970
and if that's the case, do you, warrant this discussion
423
00:24:54.049 --> 00:24:56.950
for that eventual child when they turn 18 about their
424
00:24:56.990 --> 00:24:59.440
own personal risk? Or do you try to, you know,
425
00:24:59.480 --> 00:25:02.880
quote unquote, keep it simple where it's anything under 27
426
00:25:02.880 --> 00:25:05.059
is good to go. We don't want to deal with,
427
00:25:05.240 --> 00:25:09.440
you know, anything 27 or above. These things are conversations
428
00:25:09.519 --> 00:25:11.400
that you're going to want to have if you're talking
429
00:25:11.460 --> 00:25:15.769
about genetic testing and what to move forward with and
430
00:25:15.809 --> 00:25:18.390
what not to move forward with. Yeah. Yeah.
431
00:25:19.109 --> 00:25:20.509
And so I think we could wrap up the show,
432
00:25:20.529 --> 00:25:22.650
but I'm going to do one more question. It's one
433
00:25:22.690 --> 00:25:24.910
I have not prepared Wes for, so he's going to get,
434
00:25:24.930 --> 00:25:31.630
I love doing this sometimes. How do you handle when
435
00:25:31.710 --> 00:25:34.930
you're giving results back and you've listened to this whole
436
00:25:34.970 --> 00:25:40.019
family history and then you see two positive numbers sitting
437
00:25:40.079 --> 00:25:43.000
on your screen, maybe one in positive and one in
438
00:25:43.039 --> 00:25:46.220
the gray zone. And now you know that there's possibly
439
00:25:46.279 --> 00:25:49.420
another side of the family that's also now being diagnosed?
440
00:25:50.339 --> 00:25:52.500
Yeah, that is a really tricky one, Katie. What a
441
00:25:52.559 --> 00:25:56.819
great question. I'm glad you asked that. That does happen.
442
00:25:57.099 --> 00:26:00.450
That does come up and it is tricky. It is
443
00:26:00.630 --> 00:26:04.609
most often, I think in my experience, most often the
444
00:26:04.630 --> 00:26:10.250
case that someone would have a positive result a positive allele,
445
00:26:10.289 --> 00:26:12.349
a positive copy of the gene where it's 40 or
446
00:26:12.390 --> 00:26:18.150
above and a intermediate copy where it's somewhere between 27
447
00:26:18.150 --> 00:26:22.799
to 35. And that's tricky because when someone is receiving
448
00:26:22.839 --> 00:26:26.339
that result, the way that I deliver those results is
449
00:26:26.619 --> 00:26:32.200
generally reading the two CAG repeat numbers and then describing
450
00:26:32.259 --> 00:26:35.539
what they mean. And so like, let's say that someone
451
00:26:36.049 --> 00:26:38.930
were to receive a result where their CAG was somewhere,
452
00:26:38.950 --> 00:26:42.990
let's say it was a 42 and a 17. I
453
00:26:43.029 --> 00:26:46.799
would say your CAG repeats are 42 and 17, meaning
454
00:26:46.839 --> 00:26:49.160
that you have inherited a copy of the gene that
455
00:26:49.200 --> 00:26:55.880
causes Huntington's disease. That already is a heavy result to receive.
456
00:26:56.240 --> 00:27:00.900
When you adjust that to be, your CAG repeat numbers
457
00:27:00.980 --> 00:27:04.759
are 42 and 31, meaning you've inherited a copy of
458
00:27:04.799 --> 00:27:10.019
the gene that causes Huntington's disease, that CAG of 42,
459
00:27:10.019 --> 00:27:12.400
but you've also inherited a copy of the gene that
460
00:27:12.480 --> 00:27:17.549
is in what we call the intermediate range at 31.
461
00:27:17.549 --> 00:27:20.670
This is the lower end of the gray area, meaning
462
00:27:20.730 --> 00:27:24.269
we're not really concerned that that would cause Huntington's disease,
463
00:27:24.710 --> 00:27:26.900
but it is possible for this copy of the gene
464
00:27:26.920 --> 00:27:29.740
to maybe change when passed to offspring, meaning it could
465
00:27:30.220 --> 00:27:33.839
get larger. That's a tough one because basically someone is
466
00:27:35.420 --> 00:27:39.140
dealing with receiving that positive result and then immediately put
467
00:27:39.160 --> 00:27:42.329
in a place where they're like, wait, what? So I have,
468
00:27:43.309 --> 00:27:45.849
I have a copy of the gene that causes Huntington's,
469
00:27:45.890 --> 00:27:48.750
but I also have another one and it ends up,
470
00:27:49.049 --> 00:27:52.869
you know, I think probably expectedly turning into a longer
471
00:27:52.930 --> 00:27:57.420
conversation where I then try to say, you know, upfront,
472
00:27:57.460 --> 00:27:59.779
those are the numbers. This is what it means. Let's
473
00:27:59.839 --> 00:28:03.359
back up. Let's talk about the 42. Let's talk about
474
00:28:03.380 --> 00:28:07.019
what that means. Let's talk about real quick what that
475
00:28:07.019 --> 00:28:10.660
31 means, but let's focus on the 42. Okay. Now
476
00:28:10.680 --> 00:28:12.559
we've kind of got to a place where the 42
477
00:28:12.559 --> 00:28:16.559
makes sense. Now let's really talk about the 31, you know,
478
00:28:16.660 --> 00:28:19.440
and try to get into that. And I, I think
479
00:28:19.480 --> 00:28:22.599
we do the best that we can when we're in
480
00:28:22.640 --> 00:28:26.220
that situation, but I think that it really, it maybe
481
00:28:26.299 --> 00:28:31.920
doesn't sink in until we have the follow-up after results appointment,
482
00:28:31.940 --> 00:28:34.789
maybe a week or two weeks later. And, Depending on
483
00:28:34.849 --> 00:28:37.970
if you're a genetic counselor at a clinic or you
484
00:28:38.019 --> 00:28:42.000
do this differently or whatever the case is, your after
485
00:28:42.079 --> 00:28:46.619
results follow up experience may be different. But I think
486
00:28:46.680 --> 00:28:49.240
it's worth like, let's say that you are someone in
487
00:28:49.289 --> 00:28:54.809
this situation personally. and you didn't get that from your provider,
488
00:28:55.029 --> 00:28:58.640
it's worth reaching out to them, you know, again, later
489
00:28:58.920 --> 00:29:00.920
to talk more about it and what it means, or
490
00:29:01.000 --> 00:29:03.880
even you can reach out to me at HD genetics.
491
00:29:03.960 --> 00:29:08.849
I'm happy to talk to you through it. Basically, it's just, it's,
492
00:29:09.450 --> 00:29:12.109
it can be a lot of a mental load for
493
00:29:12.150 --> 00:29:15.089
someone to, to really understand those two different things.
494
00:29:16.049 --> 00:29:16.269
Yeah.
495
00:29:16.309 --> 00:29:18.059
Great question, Katie. Yeah.
496
00:29:18.140 --> 00:29:20.980
And I have a couple of friends that this has
497
00:29:21.019 --> 00:29:23.400
happened to. That's why I brought this up. Interestingly enough,
498
00:29:23.440 --> 00:29:25.720
two of my friends, it was their child being diagnosed
499
00:29:25.740 --> 00:29:29.529
with JHD is when they learned, both of them. And
500
00:29:29.589 --> 00:29:33.750
now they're friends and they talk often. But if this
501
00:29:33.769 --> 00:29:36.700
does happen, definitely, like Wes said, reach out to him,
502
00:29:36.750 --> 00:29:39.670
reach out to a genetic counselor. But also you can
503
00:29:39.690 --> 00:29:42.180
reach out to the nonprofits because we do actually have
504
00:29:42.940 --> 00:29:45.779
people that they're going through this and they're walking this journey.
505
00:29:45.859 --> 00:29:48.400
So we can connect you guys to talk one on one.
506
00:29:48.420 --> 00:29:51.769
I know the two women, my dear friends that this
507
00:29:51.809 --> 00:29:54.430
happened to. They are such wonderful advocates and they would
508
00:29:54.470 --> 00:29:56.430
love to talk to you if you need anything at all,
509
00:29:57.089 --> 00:29:59.450
especially if your parent that is in the gray zone
510
00:29:59.470 --> 00:30:01.109
that would have never guessed. That's kind of what they
511
00:30:01.150 --> 00:30:04.900
went through. Right. So definitely always reach out to your community.
512
00:30:04.940 --> 00:30:09.039
It's amazing what supports we have out there for sure.
513
00:30:09.480 --> 00:30:12.400
So I think we could wrap up the show for now.
514
00:30:12.509 --> 00:30:15.809
Thank you so much, Wes, for coming on. And again,
515
00:30:15.890 --> 00:30:17.829
thank you to all of our sponsors for making Health
516
00:30:17.849 --> 00:30:20.470
for HD Live possible. We will see you guys. It
517
00:30:20.490 --> 00:30:22.769
looks like we are going into the holidays. So this
518
00:30:22.829 --> 00:30:26.450
whole month was on genetic testing. And then next month,
519
00:30:26.490 --> 00:30:29.329
we will be jumping into the holidays and the grief
520
00:30:29.410 --> 00:30:32.150
and hard times of the holidays, but also the joys
521
00:30:32.190 --> 00:30:33.890
we can find in it and being with the ones
522
00:30:33.930 --> 00:30:37.089
we love. So that will be in November. So tune
523
00:30:37.130 --> 00:30:40.089
in for the monthly lineup. The monthly lineup should come
524
00:30:40.130 --> 00:30:42.930
out soon. Until next time, everyone take care.
